The LTSS (Long-Term Services and Supports) Service Coordinator - Clinician role at Elevance Health is a vital field-based position predominantly operating within San Jacinto County, Texas. This role requires the candidate to reside within this area and be willing to travel up to one hour daily to client sites or other designated work locations. The LTSS Service Coordinator - Clinician works directly under the supervision of a Registered Nurse (RN) or Licensed Social Worker (LSW), holding overall responsibility for coordinating the case management and care of assigned members in compliance with applicable state laws and contracts. This role involves contributing to the LTSS care coordination process by performing activities aligned with the scope of licensure. Examples of these activities include assisting the RN or LSW with comprehensive assessments conducted either telephonically or face-to-face to identify, evaluate, coordinate, and manage member needs across physical health, behavioral health, social services, and long-term care support domains.

This position plays a significant role in supporting the responsible RN or LSW in identifying members at risk for high complication levels and gathering essential clinical data as directed. The Coordinator also helps determine which members could benefit from alternative care levels or waiver programs. All information collected by the Coordinator is reviewed and interpreted by the supervising RN or LSW, who carries out further assessments if necessary and then develops, monitors, evaluates, and revises personalized care plans to address each member's distinct needs effectively. Furthermore, the Coordinator actively participates in coordinating care for members with chronic illnesses, co-morbid conditions, or disabilities. This coordination is a collaborative effort involving the RN or LSW, the member themselves, and the broader healthcare team to ensure care delivery is cost-effective, efficient, and aligned with each member’s health goals.

The role demands solid decision-making skills based on understanding disease processes, clinical guidelines, and terminology, though it does not require independent nursing judgment. The LTSS Service Coordinator - Clinician position is ideal for candidates holding an LSW, LCSW, LMHC, or LMSW license or a similar license recognized under state law, supplemented by relevant experience with individuals managing chronic illnesses or disabilities. Job Requirements

  • Reside in San Jacinto County, Texas
  • Be willing to travel up to 1 hour daily
  • Current, unrestricted LSW, LCSW, LMHC, LMSW or other license as allowed by state law
  • Minimum of 2 years experience working with individuals with chronic illnesses, co-morbidities, or disabilities in a service coordinator or similar role
  • Ability to perform face-to-face and telephonic assessments
  • Knowledge of disease processes and clinical guidelines
  • Ability to work in a field-based role with occasional office attendance

Job Qualifications

  • LSW, LCSW, LMHC, LMSW or other applicable license other than RN as allowed by state law
  • Nursing diploma or associate degree in nursing or related field with minimum 2 years experience in service coordination or similar role
  • Experience working with individuals with chronic illnesses, co-morbidities, or disabilities
  • Ability to collaborate effectively with healthcare teams
  • Strong communication and assessment skills
  • Valid license in applicable state(s)

Job Duties

  • Assist responsible RN or LSW in identifying members for high risk complications
  • Obtain clinical data as directed by the responsible RN or LSW
  • Assist the responsible RN or LSW in identifying members that would benefit from an alternative level of care or other waiver programs
  • Provide all collected information to the responsible RN or LSW for verification, interpretation, and care plan development
  • Participate in coordinating care for members with chronic illnesses, co-morbidities, and disabilities in conjunction with RN or LSW, member, and health care team
  • Apply decision making skills based on current member needs, disease processes, and clinical guidelines
  • Travel to client sites or designated locations as required
