We are currently seeking a highly motivated LTSS Service Coordinator who is bilingual in Spanish and English to join our team. This position operates primarily as a field-based role, requiring travel to client sites within Broward County to conduct assessments and coordinate services. Candidates should reside within a reasonable commuting distance from Broward County, as the company follows a hybrid/virtual work policy requiring some in-office attendance for meetings and training. This full-time role is scheduled from Monday to Friday, 8am-5pm EST.
The LTSS Service Coordinator plays a vital role in managing the care coordination functions for a defined caseload within specialized programs. The primary responsibilities include facilitating the Person Centred Planning process in collaboration with the individuals supported, addressing their preferences, needs, and self-identified goals. This role requires conducting comprehensive assessments, developing tailored Person Centered Support Plans (PCSP), creating backup plans, and interacting with Medical Directors and interdisciplinary care teams. The coordinator ensures that individuals receive integrated care plans addressing their physical health (PH), behavioral health (BH), and Long-Term Services and Supports (LTSS) needs in compliance with state laws, federal regulations, and contract requirements.
This position offers an opportunity to make a significant impact by being the single point of contact for members, enabling smooth communication and coordination among clinical healthcare teams and support networks. Duties include performing face-to-face program assessments using structured tools and motivational interviewing techniques, identifying members with high-risk clinical needs, coordinating cases with healthcare professionals, managing non-clinical needs for individuals with chronic illnesses or disabilities, and documenting service goals collaboratively with members and their support teams. Additionally, the LTSS Service Coordinator may mentor new staff, engage in process improvement initiatives, submit utilization and authorization requests aligned with care plans, report critical incidents to appropriate agencies, and assist with member grievances, appeals, and audits.

Job Requirements
- BA/BS degree or equivalent combination of education and experience
- minimum 2 years experience working with a social work agency or in a related healthcare field
- residency within Broward County or reasonable commuting distance
- ability to travel to client sites as required
- capability to work a Monday to Friday schedule from 8am to 5pm EST
- compliance with Elevance Health vaccination policies
- authorization to report to appropriate internal and external parties such as state and county agencies
- strong organizational, communication, and interpersonal skills
- familiarity with state law and contract requirements applicable to LTSS service coordination
Job Qualifications
- Bachelor’s degree in social work, health care related field, or a related discipline preferred
- bilingual in English and Spanish strongly preferred
- experience in LTSS, case management, social work, or hospital discharge planning preferred
- knowledge of person centered planning and interdisciplinary care coordination
- strong communication and motivational interviewing skills
- ability to manage a caseload and coordinate multiple service providers
- experience working with individuals with chronic illnesses, disabilities, or behavioral health needs
- proficiency in documentation and reporting of care plans and service outcomes
Job Duties
- Perform face to face program assessments using various tools with pre-defined questions
- apply motivational interviewing techniques for evaluations, coordination, and management of waiver, behavioral health, and physical health needs
- identify members with potential clinical healthcare needs including high-risk complications and address gaps in care
- serve as the single point of contact coordinating cases with clinical healthcare management and interdisciplinary teams
- manage non-clinical needs of members with chronic illnesses, co-morbidities, and disabilities to ensure efficient utilization of long-term services and supports
- document short and long-term service and support goals in collaboration with members and their care teams including caregivers, family, providers, and physicians
- identify members that may benefit from alternative levels of service or different waiver programs
- mentor, train or serve as a subject matter expert for new staff and participate in process improvement initiatives
- submit utilization and authorization requests supporting care plans
- report critical incidents to internal and external parties including Adult Protective Services and Law Enforcement
- assist in appeals, member grievances, and state audits
