The LTSS Service Coordinator (Bilingual in Spanish/English) role at Elevance Health is primarily a field-based position with responsibility for a defined caseload of individuals enrolled in specialized Long-Term Services and Supports (LTSS) programs. This role is based in the counties of Indian River, Martin, or Palm Beach County, with the candidate required to reside in one of these locations or within commuting distance to the office. The position operates Monday through Friday from 8am to 5pm EST, offering the ability to work directly with clients in their communities and providing a flexible, hands-on approach to service coordination.
As an LTSS Service Coordinator, the associate will facilitate the Person Centered Planning process, collaborating with individuals to assess their needs, preferences, and goals. This role requires engaging with multiple stakeholders including Medical Directors, interdisciplinary care teams, caregivers, family members, and natural supports to create and manage comprehensive care plans. The coordinator will address physical health, behavioral health, and LTSS needs, ensuring compliance with relevant state laws and contracts as well as federal requirements. Additionally, this position performs face-to-face program assessments using motivational interviewing techniques, identifies members with potential clinical health care needs, and acts as the single point of contact for coordination between members and clinical healthcare management.
This role is critical in managing both clinical and non-clinical needs of members living with chronic illnesses, disabilities, and co-morbidities to optimize the utilization of services efficiently and cost-effectively. An important part of the position involves documentation and submission of utilization authorizations and coordination of critical incident reporting to appropriate agencies, including Adult Protective Services and Law Enforcement. Furthermore, the LTSS Service Coordinator may participate in appeals and grievance processes and contribute to staff training, mentorship, and process improvement initiatives.

Job Requirements
- BA/BS degree or equivalent combination of education and experience
- Minimum 2 years experience working with a social work agency or related field
- Able to operate primarily in a field-based role with some office attendance
- Reside in Indian River, Martin, or Palm Beach County or within commuting distance
- Must be bilingual in English and Spanish
- Knowledge of relevant state law and contract requirements
- Valid driver’s license and reliable transportation
- Ability to handle sensitive and confidential information
- Compliance with vaccination policies in patient/member-facing roles
Job Qualifications
- BA/BS degree or equivalent experience in social work or healthcare related field preferred
- Bilingual in English and Spanish strongly preferred
- Experience in LTSS, case management, social work or hospital discharge planning preferred
- Strong interpersonal and motivational interviewing skills
- Ability to collaborate effectively with interdisciplinary teams
- Knowledge of state and federal LTSS regulations
- Experience in documentation and utilization management
Job Duties
- Perform face to face program assessments using motivational interviewing techniques for evaluations and management of individual waiver and behavioral or physical health needs
- Identify members with potential clinical health care needs using tools and predefined processes and coordinate cases serving as single point of contact with clinical healthcare management and interdisciplinary teams
- Manage non-clinical needs of members with chronic illnesses, co-morbidities, and disabilities ensuring efficient utilization of long-term services and supports
- Document member’s short and long-term service and support goals in collaboration with their care team including caregivers and physicians
- Identify members who may benefit from alternative levels of service or other waiver programs
- Serve as mentor or preceptor for new staff and assist in formal training and process improvement initiatives
- Submit utilization and authorization requests with supporting documentation aligned with individual care plans
- Report critical incidents to appropriate internal and external parties such as state and county agencies
- Assist and participate in member grievance, appeals, and state audits processes
