The role available at Easterseals Southern California focuses on assisting individuals in group home settings by planning and conducting daily activities in participants' homes. This position is designed for compassionate and dedicated individuals who are eager to contribute to the development of vital living skills in people with disabilities. The successful candidate will work closely with participants to facilitate their independence and integration into the community in alignment with their Individual Service Plans (ISP) or Individual Program Plans (IPP). Responsibilities will include assisting participants with household chores, meal preparation, grocery shopping, transportation, medical appointments, financial management, and personal care. This hands-on role requires a balance of empathy, patience, and professionalism as well as competence in handling personal care duties such as hygiene assistance and bedtime preparation. The position also demands diligent documentation and participation in assessments, planning, and ISP/IPP meetings.

Job Requirements

High school diploma, GED, or national equivalent
1 to 2 years of applicable experience or equivalent combination of training and education
Must be at least 18 years old
Certified in non-violent crisis intervention such as CPI, PART, or MAB (ongoing certification required where applicable)
Valid CPR and First Aid certifications
Ability to provide personal care and safely transfer participants without weight-bearing skills
Capacity to lift up to 50 pounds repetitively
Ability to walk, stand, stoop, and kneel for extended periods
Valid California driver's license and clean driving record if local travel is required
Ability to tolerate varied outdoor weather conditions
Ability to pass drug testing required by Easterseals Southern California
Ability to obtain required vaccinations or provide evidence of immunity

Job Qualifications

High school diploma, GED, or national equivalent
Certified in CPR and First Aid
Knowledge of Individual Service Plans (ISPs) and Individual Program Plans (IPPs)
Effective oral and written communication skills
Ability to work independently while providing reliable support
Experience working with people in behavior management programs preferred
Ability to obtain and maintain criminal record/fingerprint clearance
Ability to pass post-offer medical examination and tuberculosis testing

Job Duties

Plan and prepare a weekly schedule of activities to support learning of living skills according to participants' ISP and IPP goals
Conduct daily activities in participants' homes to assist in developing living skills and community integration
Provide one-on-one personal care including assistance with eating, personal hygiene, showering, restroom use, and bedtime preparation
Maintain detailed documentation including ISPs, evaluations, and case notes
Assist in participant assessments, planning, and evaluation processes and attend ISP/IPP meetings
Complete required training hours using internal and external resources
Perform additional duties as assigned
