The role of Licensed Practical Nurse (LPN) at Cottage Grove Place is pivotal in delivering direct nursing care and coordinating with support staff to ensure the best outcomes for residents. This position is available in several shifts including part-time 1st shift (6:00am-2:30pm) and both part-time and full-time 2nd shift options with weekend hours available, offering flexibility to fit varied schedules. The LPN is responsible for assisting residents with activities of daily living (ADLs), supporting family and social activities, and promoting the independence of each resident. A critical part of the role involves identifying any medical or behavioral changes in residents, developing and continually monitoring care plans, and reassessing residents to ensure their evolving needs are met effectively. The LPN works closely under the direction of the Director of Nursing, providing daily reports on community conditions, resident health, personnel performance, and required nursing supplies.
In this capacity, the LPN administers nursing care as ordered by physicians adhering to recognized nursing practices, established standards, and administrative policies. The role extends to ensuring residents receive preventative, therapeutic, and rehabilitative services, along with coordinating care efforts across various disciplines to maintain comprehensive resident wellness. Job Requirements
- Graduation from accredited nursing program
- Current LPN license with the Iowa Board of Nursing
- CPR certification
- Minimum two years' experience in long term care preferred
- IV Therapy certification preferred
- Availability for emergency situations within the community
Job Qualifications
- Graduation from accredited nursing program

- Currently CPR certified
- Minimum two (2) years’ experience in long term care preferred
- Current IV Therapy Certified preferred
- Willingness to be available for any/all emergencies regarding the community
Job Duties
- Assist residents with activities of daily living (ADL), family/social activities and actively promote independent life-style of residents
- Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident
- Provide daily report to the Director of Nursing concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies
- Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies
- Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines
