The Licensed Clinical Social Worker (LCSW) or experienced Master of Social Work (MSW) position at NorthBay Health in Fairfield, CA is a full-time, weekday role integral to the behavioral health team. This role supports inpatient units and the Emergency Department, addressing a broad spectrum of mental health needs. Weekend and holiday coverage is required on a planned basis to ensure continuous support. While supervision for LCSW hours is currently not available, this opportunity is ideal for clinicians passionate about providing comprehensive mental health services, care coordination, and support for patients facing complex psychosocial challenges across the healthcare continuum. The role involves direct clinical interventions, discharge planning, and case management, working collaboratively with interdisciplinary teams to enhance patient outcomes. Job Requirements

  • Master of Social Work degree from an accredited program
  • California BBS Licensed Clinical Social Worker credential
  • Minimum two years experience in health or community setting
  • Strong clinical assessment skills
  • Excellent communication skills including verbal, written, and listening
  • Ability to work independently with minimal supervision
  • Strong critical thinking and decision-making skills
  • Ability to organize multiple tasks and meet deadlines
  • Ability to work effectively with diverse populations
  • Knowledge of relevant social work laws and ethical standards
  • Basic word processing skills
  • Willingness to participate in weekend and holiday coverage as needed

Job Qualifications

  • Master of Social Work from a school accredited by the Council on Social Work Education
  • California BBS Licensed Clinical Social Worker (LCSW) with CCS paneling required for pediatric/NICU population
  • Minimum of two years experience in acute health care or community health settings
  • Strong clinical assessment skills
  • Excellent verbal, written, and listening communication skills
  • Ability to work diplomatically and independently with minimal direction
  • Strong critical thinking and clinical decision-making skills
  • Excellent organizational and follow-through abilities
  • Ability to work effectively with diverse populations
  • Knowledge of human behavior, psychotherapy, case management, and relevant social work laws including Tarasoff and HIPAA
  • Skilled at supporting patients and families coping with loss and grief
  • Knowledge of suicidal behavior assessment and treatment planning using DSM V
  • Proficiency with word processing and experience with Cerner and PowerPoint preferred
  • Familiarity with acute, subacute, long-term care, home care, hospice, and community mental health resources
  • Ability to apply clinical theory in case management and psychotherapy
  • Demonstrated interpersonal skills including advocacy, relationship building, and respect for diverse perspectives

Job Duties

  • Provide direct clinical social work services including counseling, discharge planning, and case management for hospital patients and families
  • Perform comprehensive biopsychosocial assessments addressing emotional, psychological, social, financial, environmental, and follow-up referral needs
  • Support Emergency Department mental health cases including safety planning and intervention with training available for 5150 certification
  • Initiate and discontinue 5150 involuntary psychiatric holds consistent with legal guidelines
  • Manage psychosocially complex patients across the continuum of care by developing and monitoring care plans and addressing barriers to discharge
  • Collaborate with physicians, nurses, case managers, and interdisciplinary teams to ensure coordinated, patient-centered care
  • Facilitate integration with community-based resources and external agencies to optimize health outcomes
  • Communicate key patient information effectively across care transitions
  • Document all assessments and interventions accurately in the electronic medical record
  • Ensure compliance with NorthBay Health’s mission, policies, and regulatory standards
