This position is within the Community-University Health Care Center (CUHCC), a comprehensive health center providing medical, dental, and behavioral health services to nearly 12,000 racially and culturally diverse patients each year. CUHCC's mission is to transform care and education to advance the health of patients and their communities. The center integrates primary care, behavioral health, and dental services with a strong focus on whole-person care. Behavioral health at CUHCC includes outpatient psychiatry, therapy, substance use disorder services, and integrated behavioral health-care coordination embedded within primary care. Community programs such as Adult Rehabilitative Mental Health Services (ARMHS) and Targeted Case Management support patient wellness beyond clinical settings.
The Integrated Care & Community Health Program Manager role at CUHCC is pivotal for advancing integrated behavioral health and community health services within a multidisciplinary primary care setting. Reporting directly to the Mental Health Director, this role offers dual clinical and programmatic leadership over CUHCC’s Integrated Behavioral Health-Care Coordination (IBH-CC) team and the Community Health Team, which includes Community Health Workers (CHWs). The manager is responsible for providing direct patient care, operational oversight, and clinical supervision, ensuring that high-quality, trauma-informed, and culturally grounded behavioral health and community health services are delivered effectively. This leadership position involves managing a diverse team that includes licensed clinicians, care coordinators, CHWs, and clinical trainees.
Key clinical responsibilities include providing traditional outpatient therapy, integrated behavioral health services in primary care and dental settings, and serving as the primary clinical responder in crisis situations. The Program Manager also collaborates closely with interdisciplinary teams, such as primary care providers, dentists, nutritionists, and pharmacists, to deliver comprehensive patient-centered care. Operational duties include managing clinical staff and care coordination teams, overseeing team productivity, and leading quality improvement initiatives. The role demands strong leadership skills with experience in program development, staff supervision, and data-driven decision-making, as well as the ability to integrate trauma-informed and culturally responsive care approaches.
Furthermore, the Program Manager oversees community outreach and engagement activities by supervising the CHW team, facilitating collaboration across various CUHCC service lines, and ensuring alignment with organizational quality standards and performance metrics. They also work closely with the Director of Advancement to coordinate external partnerships and community events. This is a hybrid position based primarily on site at CUHCC with some remote work options. The environment requires adherence to health safety protocols including vaccinations and testing.

Job Requirements
- Masters degree plus at least four years of experience
- Independently licensed mental health provider per state statute
- Board-certified clinical supervisor
- Experience managing community health worker programs, care coordination services, or multidisciplinary teams
- Demonstrated ability to meet clinical productivity and program performance targets
- Ability to work onsite during clinic hours with hybrid remote work option
- Negative TB test and Hepatitis B vaccination required
- Ability to collaborate effectively with interdisciplinary teams
- Strong leadership and communication skills
- Commitment to culturally responsive and trauma-informed care

Job Duties
- Provide outpatient therapy services under the Outpatient Therapy program
- Provide integrated behavioral health clinical services in primary care and dental settings
- Serve as a primary clinical responder for crisis situations, conducting risk assessments
- Collaborate with primary care providers, dentists, pharmacists, nutritionists, CHWs, and care coordinators
- Manage the IBH-CC team and internship/practicum programs including hiring and training
- Provide clinical supervision and consultation on high-risk cases
- Oversee the care coordination team to support patients' social and medical needs
- Monitor team performance, generate reports, and lead quality improvement initiatives
- Manage the CHW team including hiring, training, and performance management
- Oversee community outreach and patient support activities by CHWs
