In-home Traveler Physician (Full-Time) - New York State and Surrounding Markets

Signify Health, a division of CVS Health, plays a vital role in this mission by providing innovative healthcare services that bring care directly to patients' homes. Signify Health engages with patients through in-home health evaluations conducted by physicians who travel extensively to meet patients where they are. This full-time Physician role focuses on delivering personalized in-home health assessments, helping bridge gaps in care by connecting individuals with the right resources and health plans. Physicians in this role spend up to 100% of their time traveling within a specified coverage area, which may include overnight stays and multiple state licenses depending on business needs.

The role is sign-on bonus eligible and offers a competitive pay range of $175.00 per visit. It involves intimate patient interactions, usually lasting about 45 minutes, where physicians conduct medication and medical history reviews, physical evaluations, and, if ordered by health plans, additional diagnostic tests such as diabetic eye exams and spirometry. Physicians are provided with an iPad and an intuitive clinical workflow to facilitate these evaluations, with no requirement to prescribe medications or alter existing treatment plans.

Physicians in this role have the opportunity to join a national network of purpose-driven clinicians dedicated to improving health outcomes and providing care that meets patients' unique needs in the comfort of their own homes. Responsibilities may also include conducting virtual visits and providing diagnostic and preventive services. This unique work environment requires adaptability and comfort with diverse home settings that may have household pets, secondhand smoke, or other allergens. This position requires active, unrestricted medical licenses in the coverage area and the ability to obtain and maintain multiple state licenses as necessary. Coverage areas primarily include all counties within New York State, including Albany, Binghamton, Buffalo, Elmira, NYC, Plattsburgh, Poughkeepsie, Rochester, Syracuse, Utica, and Watertown, with flexibility to cover other areas as business needs dictate. A dedicated concierge-style Network Activation team supports physicians with licensing requirements.
