Within the CVS Health family, Signify Health plays a critical role by delivering personalized in-home health evaluations to patients, enabling better access to care in the comfort of their own homes. The company is currently seeking a full-time Clinician, specifically a Nurse Practitioner (NP) or Physician Assistant (PA), to join their dedicated network of healthcare professionals. In this role, the clinician will primarily travel up to 100% of the time within a designated coverage area to provide comprehensive health assessments directly at patients’ homes. This approach not only bridges gaps in health care access but also creates a more personalized and patient-centered experience.
The full-time clinician position involves performing visits approximately 45 minutes in length, which include medication and medical history reviews, physical evaluations, and additional diagnostic tests when ordered by the patient’s health plan. The diagnostic services may encompass diabetic eye exams, spirometry, and bone density screenings, among others. Using an iPad and an intuitive clinical workflow, clinicians are supported with technology to guide their assessments. Importantly, clinicians are not responsible for prescribing medication, ordering lab tests, or changing current treatment regimens during evaluations. Instead, the role emphasizes engagement, building trusting relationships, providing education, and helping guide patients toward appropriate health care resources.
Travel demands include car rentals, flights, and overnight stays as necessary, depending on the coverage region. Multi-state licensure will be required due to the geographic breadth of the service area. This flexibility enables clinicians to cover a wide range of counties and adapt to changing business needs. Job Requirements
- Bachelor's degree in nursing or related health field
- Active nurse practitioner or physician assistant license
- Board certification
- Ability to obtain and maintain multi-state licenses
- Willingness to travel extensively and stay overnight as needed
- Comfortable working in private residences with potential allergens
- Reliable transportation and valid driver's license
Job Qualifications
- Current active and unrestricted license as nurse practitioner or physician assistant in coverage area
- Board certification as nurse practitioner or physician assistant
- Demonstrated ability to obtain and maintain multi-state licenses
- Experience working in diverse patient environments comfortably and safely
- Strong patient engagement and communication skills
- Ability to flexibly cover all counties within the state and other business needs
- Comfortable with extensive travel and use of technology for healthcare delivery
