Signify Health, a part of CVS Health, is seeking a Full-Time Clinician who is either a Nurse Practitioner (NP) or Physician Assistant (PA) to provide In-Home Health Evaluations. This role is pivotal in delivering personalized healthcare services directly to patients in their homes, helping bridge gaps in care and ensuring patients receive the right care tailored to their unique health needs. The clinician will conduct comprehensive evaluations including medication and medical history reviews and physical exams, with additional diagnostic testing if ordered by the patient’s health plan.

This full-time position requires extensive travel—up to 100% within a defined coverage area—often involving overnight stays, car rentals, or flights depending on business needs. Due to the scope of this travel, candidates must hold or be able to obtain multi-state licenses as required. The use of technology such as iPads and an intuitive clinical workflow supports clinicians in efficiently conducting evaluations without the need to prescribe medication or modify current treatment plans.

In this role, clinicians play a critical role in building trusted, long-lasting relationships with patients, allowing for in-depth health discussions in a comfortable, home setting. This approach enables a better understanding of patients' environments and health challenges, fostering more precise and relevant care recommendations.
