This full-time opportunity for a Nurse Practitioner or Physician Assistant is centered on delivering comprehensive, personalized health evaluations to individuals in their homes, ensuring that patients receive the right care in the comfort of their environment. The role involves extensive travel—up to 100%—across designated coverage areas, necessitating flexibility to stay overnight and travel by car or plane as needed. Holding licenses in multiple states may be required depending on business needs.

As a clinician at Signify Health, you will engage directly with patients during approximately 45-minute visits that encompass medication and medical history reviews, physical evaluations, and additional diagnostic tests like diabetic eye exams and spirometry if ordered by the patient’s health plan. Utilizing technology such as iPads and an intuitive clinical workflow, clinicians do not prescribe medication or alter existing treatment plans, allowing them to focus fully on patient engagement and assessment.
