The Full-time Clinician role, available through Signify Health which is part of CVS Health, offers a unique opportunity to engage directly with individuals in the comfort of their own homes while playing a vital role in bridging gaps in healthcare. This position involves providing in-home health evaluations primarily as a Nurse Practitioner or Physician Assistant. The clinician travels extensively—up to 100% of the time—covering defined areas, potentially requiring overnight stays, car rentals, or flights. Holding active and unrestricted multi-state licenses is essential to meet business requirements. These evaluations, lasting about 45 minutes, include comprehensive medication and medical history reviews, physical assessments, and possible additional diagnostic tests such as diabetic eye exams and spirometry as ordered by the individual's health plan.

This role prioritizes personalized care, allowing clinicians to connect one-on-one with patients, addressing health questions and helping to manage their well-being in a meaningful, patient-centered way. The clinician uses modern technology including iPads and an intuitive clinical workflow to conduct evaluations. The role does not involve prescribing medications or altering existing treatment plans but focuses on thorough assessments and facilitating connections to appropriate care pathways.

Clinicians in this role join a national network dedicated to improving health outcomes by visiting people in their homes, emphasizing a compassionate, flexible, and comprehensive approach. Additionally, clinicians receive per-completion incentives for all in-home visits, and the work environment predominantly consists of private homes that may have varied conditions including pets and environmental allergens. The role is designed for clinicians who are adaptable and comfortable working in diverse home environments.
