The role at hand is a Full-time Clinician position, specifically for Nurse Practitioners or Physician Assistants, working with Signify Health under the CVS Health umbrella. This clinician role is unique in that it is fully travel-based, requiring clinicians to visit patients in their homes across designated coverage areas. The job entails up to 100% travel, which may include overnight stays, car rentals, and flights to cover locations primarily within the state of New Hampshire and potentially other business-defined areas. Clinicians will need to hold multiple state licenses as business demands dictate. This is a sign-on bonus eligible role, emphasizing the company's drive to attract skilled professionals who are passionate about making significant impacts on patient health outcomes.

Clinicians in this role conduct in-home health evaluations lasting approximately 45 minutes, giving ample time to engage with patients one-on-one, answer health questions, review medications and medical histories, and perform physical evaluations. If ordered by the patient’s health plan, additional diagnostic tests such as diabetic eye exams and spirometry may also be conducted. These evaluations are facilitated using modern technology, including an iPad integrated with an intuitive clinical workflow. Importantly, the role does not require clinicians to prescribe medications, order lab tests, or modify existing treatment plans, focusing instead on comprehensive assessments and patient engagement.
