This role is for a Full-time Clinician, either a Nurse Practitioner or Physician Assistant, within Signify Health, which is part of CVS Health. The role centers on providing in-home health evaluations, offering personalized and comprehensive health assessments in the comfort of patients’ homes. As a traveling clinician, you will connect with patients across a designated coverage area, which may require extensive travel including overnight stays, car rentals, and flights. Holding multiple state licenses may be necessary to meet business needs. The job offers a unique opportunity to build trusted relationships and impact health outcomes by delivering flexible, one-on-one evaluations that include medication and medical history reviews, physical evaluations, and additional diagnostic tests when ordered by health plans. This visit typically lasts about 45 minutes and leverages modern technology like an iPad and an intuitive clinical workflow for streamlined patient assessments. Professionals in this role do not prescribe medication or alter treatment plans but play a vital role in bridging gaps in care and ensuring patients are connected to the appropriate health services.

Job Requirements

  • Board certification required
  • Ability to obtain and maintain multi-state licenses

  • Ability to cover all counties within Massachusetts and flex to other business needs
  • Comfortable performing duties in patients’ private residences with diverse conditions
  • Eligible to work full time

Job Qualifications

Job Duties

  • Provide in-home health evaluations including medication and medical history review
  • Perform physical evaluations and additional diagnostic tests as ordered by health plans
  • Use iPad and clinical workflow tools for documentation and assessment
  • Build trusted relationships with patients to improve health outcomes
  • Travel up to 100% across designated coverage areas including possible overnight stays
  • Conduct virtual visits as needed
  • Deliver Diagnostic and Preventive Services such as spirometry and diabetic eye exams
