This full-time clinician role with Signify Health, a CVS Health company, focuses on delivering in-home health evaluations conducted by nurse practitioners or physician assistants. You will be traveling extensively, up to 100% of the time, within a designated coverage area primarily in Massachusetts, with potential overnight stays and use of rental cars or flights as needed. As an in-home clinician, your visits will last about 45 minutes and include a medication and medical history review, physical evaluations, and if required by the patient’s health plan, additional diagnostic testing such as diabetic eye exams or spirometry. Utilizing an iPad-enabled clinical workflow, you will document assessments but are not expected to prescribe medication or alter existing treatment plans. Job Requirements
- Primary residence in Massachusetts
- Active nurse practitioner or physician assistant license
- Board certification
- Ability to travel extensively including overnight travel
- Willingness to work in diverse home environments with potential allergens
- Ability to cover all counties within Massachusetts and flex to other areas as needed
- Capacity to work full-time, approximately 40 hours per week
Job Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification as nurse practitioner or physician assistant
- Ability to obtain and maintain multi-state licenses as required
- Experience in conducting patient evaluations and assessments
- Strong communication and interpersonal skills
- Ability to work independently and manage a mobile work schedule

Job Duties
- Conduct in-home health evaluations including medication and medical history reviews
- Perform physical evaluations during visits
- Administer additional diagnostic tests as ordered by health plans such as diabetic eye exams and spirometry

- Travel extensively within the assigned coverage area up to 100 percent of the time
