Signify Health, operating under the vast umbrella of CVS Health, is seeking a passionate and dedicated Part Time Clinician, specifically Nurse Practitioners (NPs) or Physician Assistants (PAs), to deliver in-home health evaluations. This unique role offers the opportunity to engage closely with patients in their own homes, facilitating a comprehensive and personalized health experience designed to bridge gaps in care. Clinicians will conduct thorough assessments, including medication and medical history reviews, physical evaluations, and additional diagnostic tests as ordered, such as diabetic eye exams and spirometry. These visits typically last about 45 minutes, allowing plenty of time for meaningful one-on-one interactions and health education.

In this role, clinicians become trusted allies in patients' health journeys, leveraging technology like iPads and intuitive clinical workflows to streamline the evaluation process without the need for prescribing medications or altering treatment plans. Flexibility in travel within licensed states and the capacity to perform virtual visits when necessary add to the dynamic nature of this job.
