Signify Health, a part of CVS Health, is seeking dedicated healthcare professionals for the role of Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role is perfect for clinicians who have a passion for engaging with people and guiding them on their journey to better health within the comfort of their own homes. As a Part-time Clinician, you will play a vital role in building trusted relationships with health plan members to improve health outcomes through flexible, comprehensive, and personalized health evaluations.

In the Part-time Clinician role, you will conduct visits lasting approximately 45 minutes, allowing for meaningful one-on-one interaction with patients. These visits include medication and medical history reviews, physical evaluations, and additional diagnostic tests such as diabetic eye exams and spirometry when ordered by the health plan. Utilizing an iPad and an intuitive clinical workflow, clinicians focus on gathering essential health information without the need to prescribe medication, order lab tests, or alter the patient’s current treatment plans. This role offers the unique opportunity to make a significant change in the communities served by helping patients understand their health better and connecting them with appropriate care resources.

Signify Health’s in-home work environment requires clinicians to perform evaluations in private residences, meeting patients where they are.
