The role of Part-time Clinician at Signify Health, a part of CVS Health, offers a unique opportunity for Nurse Practitioners (NPs) or Physician Assistants (PAs) to provide in-home health evaluations. This role is designed for passionate healthcare professionals who enjoy engaging directly with people and aim to improve health outcomes by conducting personalized, flexible, and comprehensive health assessments in the comfort of patients' homes. Candidates will work within a national network of purpose-driven clinicians, helping bridge gaps in care by delivering crucial health evaluations that include reviewing medication and medical history, performing physical evaluations, and conducting additional diagnostic tests such as diabetic eye exams and spirometry when ordered by the patient’s health plan.
Part-time clinicians will conduct visits averaging 45 minutes, allowing time for meaningful one-on-one interactions to address health questions comprehensively. The role is innovative, leveraging technology such as iPads and user-friendly clinical workflows to ensure each evaluation is thorough and patient-centered. Clinicians in this role are not expected to prescribe medications, order lab tests, or alter existing treatment plans but focus on delivering quality in-home assessments that facilitate connections to the right care pathways. The position requires flexibility in local travel as well as within licensed states and may occasionally include virtual visits.
The work environment is dynamic and involves visiting patients in their private residences, which may feature various conditions including the presence of pets or potential allergens. Candidates must be comfortable performing their clinical duties safely within such diverse settings. The role is part-time with an anticipated weekly commitment of 29.5 hours, offering a competitive salary range from $41.85 to $90.13 per hour, depending on experience, education, and geographic location.

Job Requirements
- Active, unrestricted license as a nurse practitioner or physician assistant in coverage area
- Completion of 2080 hours of post-graduate collaborative practice hours in an integrated clinical care setting
- Filing of appropriate attestation with the state of Minnesota
- Board certification required
- Ability to flexibly travel locally and within licensed states
- Comfortable working in diverse home environments including with pets and allergens
- Reliable access to transportation for travel to patient homes
Job Qualifications
- Active, unrestricted license as a nurse practitioner or physician assistant in the coverage area
- Completion of 2080 hours of post-graduate collaborative practice hours in an integrated clinical care setting
- Board certification as a nurse practitioner or physician assistant
- Multi-state licenses preferred
- Ability to work part-time hours
Job Duties
- Conduct in-home health evaluations including medication and medical history reviews
- Perform physical evaluations during patient visits
- Administer additional diagnostic tests as ordered by health plans such as diabetic eye exams and spirometry
- Use digital tools and clinical workflows to document and manage patient evaluations
- Engage and build trusted relationships with patients to improve health outcomes
- Flexibly travel to patient homes locally and within licensed states
- Conduct virtual visits when necessary
