In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Washington (Traveler)

Signify Health, a part of CVS Health, is currently seeking a Full Time Clinician, specifically a Nurse Practitioner or Physician Assistant, to join their dynamic team providing In-Home Health Evaluations. This role is centered on improving health outcomes by engaging with patients in the comfort of their own homes, conducting thorough evaluations, and addressing individual health needs directly. The clinician will travel extensively, potentially up to 100% of the time, across a defined coverage area which may include overnight stays, car rentals, and flights. The role requires multiple state licenses depending on business needs.

This unique position focuses on building trusted relationships with patients by delivering flexible, comprehensive, and personalized health services directly to their front doors. Patient visits last approximately 45 minutes and include medication and medical history reviews, physical evaluations, and potentially additional diagnostic tests such as diabetic eye exams and spirometry when ordered by the patient’s health plan. The clinician uses technology such as an iPad with an intuitive clinical workflow to facilitate evaluations but is not responsible for prescribing medicine, ordering lab tests, or modifying current treatments.

The ideal candidate holds an active, unrestricted license and board certification for their respective role within the coverage area, and must be able to obtain and maintain multi-state licenses as required. This full-time position offers an anticipated 40-hour work week and competitive pay ranging typically from $95,738 to $206,206 annually, based on experience, education, and geography.
