Job Description

CVS Health is a leading healthcare company committed to helping people on their path to better health by providing innovative and compassionate health services. As a Fortune 500 company, CVS Health operates a broad range of pharmacies, clinics, and healthcare services across the United States, serving millions of customers every day. The organization is dedicated to simplifying healthcare access and improving health outcomes for individuals, families, and communities by integrating care and delivering connected, convenient solutions. Their vision revolves around transforming the overall health experience through a commitment to quality, safety, and accountability. Employees at CVS Health enjoy a culture that values collaboration, continuous innovation, and community impact, making it an ideal place for healthcare professionals who wish to make a meaningful difference in people's lives.

Signify Health, a part of CVS Health, is actively seeking a Part-Time Clinician — either a Nurse Practitioner (NP) or Physician Assistant (PA) — to join their team providing in-home health evaluations. This role is focused on delivering personalized, comprehensive health assessments within the comfortable and familiar environment of patients' homes. The position offers a flexible part-time schedule with approximately 29.5 hours per week and a competitive pay range from $41.85 to $90.13 per hour, dependent on experience, education, and geographic location. Working as an in-home health clinician with Signify Health means joining a nationwide network of passionate professionals who are dedicated to improving health outcomes by connecting individuals to the right care based on their unique health needs.

Clinicians in this role conduct health evaluations typically lasting about 45 minutes, which include medication reviews, medical history assessments, physical exams, and additional diagnostic tests such as diabetic eye exams or spirometry when ordered by the patient's health plan. Clinicians use an iPad equipped with an intuitive clinical workflow to document visits but are not required to prescribe medication, order labs, or modify current treatment plans. Travel is required locally and may include multiple licensed states depending on the clinician's credentials. The environment is diverse, involving visits to private residences where conditions such as household pets or airborne allergens may be present, requiring clinicians to be adaptable and comfortable working in varied settings.

This position offers an opportunity to make a tangible difference in patients' lives by fostering trusting relationships, educating patients, and enabling better health management through individualized care. The role's flexibility, combined with reduced administrative burdens, allows clinicians to focus specifically on patient care and education. Part-time employees also receive select benefits including malpractice coverage during work-related activities, supplies, and other programs supporting overall well-being. This is an ideal position for healthcare providers seeking purposeful, patient-focused work in a supportive and innovative environment that values their expertise and dedication to community health.
