CVS Health is a leading integrated health services company committed to shaping a more connected, convenient, and compassionate health experience for every individual. With a team of passionate colleagues who prioritize safety, quality, and innovation, CVS Health is dedicated to simplifying healthcare for people, families, and communities nationwide. As part of its comprehensive health services, CVS Health operates Signify Health, a dynamic network focused on delivering personalized in-home healthcare evaluations. These evaluations aim to bridge gaps in care by providing flexible, comprehensive, and convenient health services directly at patients' residences.\n\nThe role of a Full-Time Clinician (Nurse Practitioner or Physician Assistant) at Signify Health involves performing in-home health evaluations where the clinician engages with individuals in their living environments. This unique position allows clinicians to establish trusted relationships and deliver personalized health assessments that include medication and medical history reviews, physical evaluations, and additional diagnostic tests when ordered by the patient’s health plan. The typical visit lasts about 45 minutes and leverages advanced technology such as iPads combined with intuitive clinical workflows, enabling clinicians to focus on patient care without the administrative burdens of prescribing medication or ordering lab tests. This role plays a critical part in improving overall health outcomes by connecting patients with the right care tailored to their unique needs.\n\nClinicians working with Signify Health embrace a meaningful mission to enhance healthcare access and quality by meeting patients where they are most comfortable—their homes. This position requires some local travel and flexibility, including conducting virtual visits when necessary, to serve a diverse member base across licensed states. The work environment is varied, involving home settings that may include pets, secondhand smoke, and other common household conditions, requiring adaptability and a commitment to patient-centered care. This full-time role offers a competitive pay range between $87,035 and $187,460 annually, reflecting factors such as experience, education, and geographical location, alongside additional reimbursement opportunities tied to diagnostic and preventative services. Full-time employees enjoy a robust benefits package, including health, dental, and vision insurance, paid time off, annual continuing education allowances, 401K plans, malpractice coverage, and other perks. By joining Signify Health, clinicians become part of a national network that values their expertise and dedication to enhancing community health, all while enjoying a supportive work environment that emphasizes meaningful patient interactions and professional fulfillment.
