ILS (Intensive Long-Term Support) LPN

The employment opportunity is for a Licensed Practical Nurse (LPN) in the Intensive Long-Term Support (ILS) program. This role is vital in delivering 24-hour residential care to adults with serious persistent mental illness (SPMI) or co-occurring disorders, who have been discharged from inpatient psychiatric treatment. The ILS program functions as an adult supportive residential facility, integrating treatment and rehabilitation services including psychiatric care, nursing support, case management, recovery services, and personal care. Staff-to-client ratios are maintained at a minimum of one staff member per eight clients at all times.

The ILS LPN plays a key role in providing direct nursing care and support within this holistic environment. This position requires a licensed practical nurse with a current Tennessee license who is dedicated to enhancing the quality of life for residents through medical assessments, medication administration, safety monitoring, and supporting the interdisciplinary team. Responsibilities include conducting medical assessments twice weekly, administering medications, documenting all care fully and accurately as per CARF and funder standards, and ensuring client safety through constant alertness during shifts. The role also involves frequent community engagement, such as accompanying clients on outings and facilitating social interaction and self-care skills training. Flexibility is essential, as the position demands availability for a 24/7 program, including on-call duties and providing coverage for staff as necessary. The LPN must maintain compliance with strict documentation and clinical protocols to support holistic treatment planning and to collaborate effectively with primary care physicians and other providers. This role requires physical stamina, including the ability to lift up to 50 pounds, push and pull up to 150 pounds, and involves active physical intervention skills including verbal and physical de-escalation techniques.
