The Hospital Outreach Manager position at DaVita plays a pivotal role in ensuring that DaVita is recognized as the premier provider for hospital dialysis services. This role demands a strategic and collaborative individual dedicated to building and maintaining strong relationships with renal discharge planners and case managers. Your primary responsibility will be to support newly diagnosed and hospitalized dialysis patients by facilitating a smooth transition from hospital to outpatient care, ensuring patients receive seamless, quality service every step of the way. Engaging with hospital staff, analyzing data insights, and communicating effectively with leadership are key components of this position. You'll partner with internal field teammates to optimize processes and implement best practices that directly impact patient care and business growth. Job Requirements

  • Bachelor's degree or equivalent experience
  • Live in Austin or San Antonio, TX
  • Ability to travel onsite up to 40% within Austin and San Antonio
  • Willingness to travel to South Texas once per quarter
  • Strong interpersonal and communication skills
  • Experience in account management, account executive, or project management highly preferred
  • Ability to work in a hybrid role
  • Excellent time management and organizational skills

Job Qualifications

  • Bachelor's degree or higher preferred
  • Experience in account management, account executive, or project management roles highly preferred
  • Strong communication and relationship-building skills
  • Proven ability to analyze data and present findings to leadership
  • Strategic thinking and problem-solving abilities
  • Experience working in a healthcare or related setting is an advantage
  • Ability to manage multiple priorities and work independently
  • Familiarity with dialysis care and hospital procedures is a plus

Job Duties

  • Engage renal discharge planners and case managers to build and strengthen relationships
  • Ensure a smooth and seamless transition experience for newly diagnosed and hospitalized dialysis patients to outpatient settings
  • Analyze data insights to highlight importance and drive growth
  • Demonstrate executive presence when delivering insights to upper leadership with clear and impactful communication
  • Perform root cause analysis and identify opportunities based on patient feedback
  • Consistently reach out to discharge planners to provide education and build awareness of DaVita offerings
  • Prioritize key insights and opportunities to align with business goals and stakeholder needs
  • Identify growth opportunities through relationship building and addressing feedback
  • Collaborate with internal field teammates to optimize the patient admission process and implement best practices
  • Translate feedback into growth-oriented business strategies using analytical rigor and strategic thinking
  • Assist with communication between hospital, admission, and clinic teams to resolve patient placement challenges
  • Support a smooth and seamless discharge experience for patients and hospital staff
  • Develop comprehensive knowledge of the local healthcare landscape
