Qualified candidates must be registered with the Ohio State Tested Nurse Aide (STNA) Registry, holding a valid STNA certification, and have a minimum of one year of clinical experience in an acute, rehabilitation, or skilled nursing setting. Additional requirements include possessing a valid Ohio driver’s license with proof of automobile insurance coverage and Basic Life Support (BLS) certification through the American Heart Association or American Red Cross, to be obtained during orientation. Preferred certifications include Hospice and Palliative Care Nurse Assistant (CHPNA) certification and prior home care experience, adding valuable skills to patient-centered care delivery.

Job Duties
- Perform basic patient care activities
- Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate
- Assist patients with tending to personal care, activities of daily living and transfers/transport
- Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff
- Assist with keeping the patient’s environment clean and orderly
- Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions
- Escalate change in patient condition to patient’s physician as appropriate
