Job Requirements
- High school diploma or GED
- State Tested Nurse Aide (STNA) certification with the State of Ohio Nurse’s Aide Registry
- One year of clinical experience as an aide in an acute, rehab or skilled nursing setting
- Valid driver’s license (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage
- Basic Life Support (BLS) certification through the American Heart Association (AHA) or the American Red Cross within orientation period
Job Qualifications

Job Duties
- Perform basic patient care activities
- Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate
- Assist patients with tending to personal care, activities of daily living and transfers/transport
- Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff
- Assist with keeping the patient’s environment clean and orderly
- Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions
- Escalate change in patient condition to patient’s physician as appropriate
