Job Requirements

  • Certified Nursing Assistant (CNA) license
  • Six months of patient care experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
  • Current driver's license and automobile insurance
  • Current CPR certification
  • Ability to provide care wherever the patient calls home
  • Compassionate, kind and empathetic towards patients and their families
  • Flexibility and composure while supporting others in times of crisis

Job Qualifications

  • Certified Nursing Assistant (CNA) license
  • Six months of patient care experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
  • Additional preferred experience such as hospital staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, nursing home or travel experience
  • Current driver's license and automobile insurance
  • Current CPR certification

Job Duties

  • Report directly to the Administrator, Executive Director or Patient Care Manager Senior and function under the direction, instruction and supervision of the Registered Nurse and/or Clinical Supervisor
  • Help the patient to maintain good personal hygiene
  • Assist in maintaining a safe and healthy environment
  • Prepare and serve light meals
  • Assist in feeding patient when needed
  • Assist the patient in ambulation
  • Assist with the safe use of a walker or wheelchair as taught and supervised by the nurse
  • Assist with light housekeeping tasks such as emptying trash and changing linens
  • Assist with certain treatments as ordered on the aide plan of care and as allowable in state-specific regulations
  • Attempt to promote patient's mental alertness through involvement in activities of interest
  • Give basic emotional and psychological support to the patient and other members of the household
  • Report any change in the patient's mental or physical condition or home situation to the Registered Nurse and members of the interdisciplinary team
  • Demonstrate basic care to the primary caregiver
  • Practice infection control measures and standard precautions
  • Provide accurate documentation with visit itineraries
  • Attend staff meetings
  • Participate in at least one Quality Improvement activity yearly, as assigned by supervisor
  • Complete documentation in a timely manner as outlined in orientation
