Job Requirements
- Must be a certified nurse’s aide (CNA)
- State Tested Nursing Assistant (STNA) certification for Ohio
- Must maintain a valid driver’s license
- Good driving record
- Reliable automobile
- High school graduate or GED preferred
- Ability to work within a multi-disciplinary team
- Ability to work in field situations
Job Qualifications
- Must be a certified nurse’s aide (CNA)
- State Tested Nursing Assistant (STNA) certification for Ohio

- Demonstrated ability to work within a multi-disciplinary team setting
- Demonstrated ability to work in field situations
Job Duties
- Provide assistance with at least 2 of the following ADL's including personal hygiene
- Assist patients with medication intake and prescribed exercises
- Perform incidental household services essential to patient health care
- Follow the Plan of Care as instructed and make appropriate updates during scheduled visits
- Ensure the use of the 4Ms (What Matters, Medications, Mentation, and Mobility) and provide Age-Friendly Care
