The role of the Medical Social Worker at CenterWell Home Health is an integral component of this interdisciplinary team. Reporting directly to the Clinical Manager, the Medical Social Worker is tasked with assessing and addressing the social and emotional needs of home health patients, tailoring individualized care plans that complement the clinical interventions provided by other healthcare professionals. This role includes participation in care coordination, evaluation of patient's emotional and social states in relation to their medical conditions, and connecting patients with critical community resources to aid in their recovery and quality of life. The Medical Social Worker also supervises the Social Worker Assistant when involved in patient care, ensuring compliance with company policies, healthcare standards, and regulatory requirements.

This position is a full-time employment opportunity, with a pay range of $65,000 to $88,600 annually, depending on geographic location, individual skills, experience, and certification levels. The Medical Social Worker is also subject to Humana's tuberculosis screening program due to the patient-facing nature of the role and must adhere to the company’s driver safety standards, including maintaining valid driver's license and insurance. The position demands flexibility in work hours and local travel to effectively manage patient visits and community resource coordination.

Job Requirements

  • Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice
  • Social Worker licensure in the state of practice, if required by state law or regulation
  • A valid driver's license, auto insurance, and reliable transportation
  • Proof of current CPR certification
  • Experience with discharge planning needs, and obtaining community resources such as housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility
  • Knowledge of medications and their correct administration
  • Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations
  • Flexibility in work hours and travel locally
  • Maintain current licensure certifications and meet mandatory continuing education requirements
  • Must read, write and speak fluent English

Job Duties

  • Assess the patients social and emotional state as it relates to their illness or injury, needs for care and their response to such treatment, and adjustments to care
  • Assess any relationships of the patients medical and nursing needs in the home setting, financial resources, and available community resources
  • Provide any appropriate action to obtain available community resources to help resolve issues that may impede the patients recovery
  • Communicate with patients and their family members, and associates in the organization
  • Provide ongoing assessment of patient and family needs and responses to understanding the treatment process and coping with social and emotional responses
  • Participate as a member of the interdisciplinary care team in care coordination activities and act as a resource to other health team members in the identification and resolution of patient needs
  • Evaluate the performance of the Social Work Assistant to assure that all medical social services are provided to patients in compliance with company, government, and professional standards
  • Maintain and submit documentation as required by the company and or facility including any case conferences, patient/physician community contacts, visit reports progress notes, and confers with other health care disciplines in providing optimum patient care
