The role of a Home Health Physical Therapist at CenterWell Home Health is pivotal in supporting patients’ recovery journeys by providing expert, patient-centered physical therapy care within the comfort of their own homes. Reporting directly to the Clinical Manager, the therapist is responsible for planning and administering prescribed treatments designed to help patients regain mobility, strength, and independence. This position empowers individuals to overcome functional limitations and achieve a better quality of life through personalized rehabilitation programs aligned with physician directives and patient goals. The therapist also plays a critical role in assessing patients’ physical conditions, documenting progress, and coordinating with care team members including PTAs and Home Health Aides to ensure consistent delivery of care. Comprehensive communication with physicians, patients, families, and interdisciplinary teams is integral to this role, ensuring that treatment plans are reported and adjusted based on patient progress.

This full-time position offers a competitive salary range from $93,000 to $128,000 per year, reflecting the expected 40-hour workweek. Additionally, compensation per visit ranges from $59 to $83. CenterWell Home Health maintains strict policies for patient safety and compliance, including tuberculosis screening and valid driver’s license requirements due to the nature of home visits. Job Requirements

  • Current and unrestricted Physical Therapy license
  • Current CPR certification
  • Experience collaborating with a team of healthcare professionals
  • Must have valid state driver’s license
  • Must maintain personal vehicle liability insurance in accordance with state minimum required limits
  • Willingness to undergo Tuberculosis (TB) screening as part of patient-facing employment requirements

Job Qualifications

  • Current and unrestricted Physical Therapy license
  • Current CPR certification
  • Experience collaborating with a team of healthcare professionals
  • One year of experience as a Physical Therapist preferred

Job Duties

  • Plan and administer prescribed physical therapy treatment for patients working onsite in their homes to attain highest level of physical function
  • Assess and document patients' physical strengths to support physician evaluation and guide treatment goals
  • Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and goals of the patient team
  • Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation
  • Document patient observations, interventions, and evaluations promptly and thoroughly
  • Report patient progress to the physician, Clinical Manager, care team, patient, and family and submit all required evaluations, plans, progress notes, and discharge summaries
