The role of the Home Health Physical Therapist Assistant at CenterWell Home Health is crucial in bridging the gap between prescribed physical therapy interventions and patient implementation in a home setting. Reporting directly to the Clinical Manager, the therapist assistant plays an instrumental role in administering individualized therapeutic treatment plans designed by licensed Physical Therapists. This position requires hands-on engagement with patients to deliver non-complex active and passive manual therapeutic exercises, therapeutic massages, and apply various modalities including traction, heat, light, cold, water, and electrical treatments. The goal of the role is to help patients regain strength, mobility, and functional independence by fostering a supportive and adaptive care atmosphere in their private homes.

This position entails meticulous documentation of client records and timely submission of clinical documentation, ensuring seamless communication within the care team and adherence to healthcare regulations. The Physical Therapist Assistant also participates in educational meetings and department in-services for continuous professional development and quality enhancement of care delivery. Maintaining and reporting the status of equipment and supplies is a vital part of the job to ensure the uninterrupted provision of therapy services.

The role requires strict adherence to infection control and universal precautions, guaranteeing patient safety and the highest standards of care. As a full-time role, it demands 40 scheduled weekly hours, reflecting a commitment to consistent and reliable care for patients. Compensation for this position is competitive, with pay ranging from $64,000 to $87,500 annually, depending on location, experience, and qualifications, supplemented by a pay-per-visit/unit rate ranging from $41 to $57.

Candidates applying for this position must have a current state license to practice physical therapy, valid driver’s license, auto insurance, reliable transportation, and current CPR certification. Experience collaborating in multidisciplinary healthcare teams and strong knowledge of the geriatric population's physical, emotional, social, and biological needs are essential. Fluency in English is also required to ensure effective communication. Preferred candidates will have at least one year of experience as a Physical Therapy Assistant. In line with Humana’s patient safety policies, this role includes mandatory tuberculosis screening and compliance with vehicle insurance requirements.
