The Home Health Physical Therapist Assistant role is a critical part of CenterWell Home Health’s care delivery team, reporting directly to the Clinical Manager. This position involves administering physical therapy treatment plans within patients' homes as created by the supervising Physical Therapist. The role demands the application of non-complex active and passive manual therapeutic exercises and various therapeutic modalities such as massage, traction, heat, light, cold, water, and electrical treatments. Accurate and timely documentation of client records and clinical information submission is essential to maintain quality care and comply with regulatory standards. The assistant also participates in departmental educational meetings and coordinates in-service training sessions to support professional development and maintain clinical competence.

Ideal candidates for this position are skilled in recognizing and managing the unique physical, emotional, social, and biological changes experienced by the geriatric population and have the ability to communicate fluently in English. The role requires a valid state license to practice physical therapy, current CPR certification, reliable transportation, and a valid driver’s license as it involves travel to patient homes. Experience working collaboratively within a multidisciplinary healthcare team enhances service quality and patient outcomes. This full-time position offers scheduled weekly hours of 40 with a pay rate between $41.00 and $57.00 per visit, translating to an annual salary range of approximately $64,000 to $87,500 depending on location and experience.
