As a Home Health Physical Therapist Assistant at CenterWell Home Health, you will be an integral part of this caring community, working under the supervision of the Clinical Manager. This full-time role requires a commitment of 40 scheduled weekly hours, paying between $64,000 and $87,500 annually based on skill, experience, location, and other qualifications. Additionally, there is a pay-per-visit/unit compensation range of $41.00 to $57.00, reflecting the direct visits and units delivered. This position involves administering personalized physical therapy treatments to patients in their homes, based on plans developed by licensed Physical Therapists. The goal is to help patients regain strength, mobility, and independence in a comfortable and familiar environment, empowering them to overcome limitations and enjoy an improved quality of life.

Key responsibilities include administering active and passive manual therapeutic exercises, therapeutic massage, and various modalities including traction, heat, light, cold, water, and electrical treatments. Accurate and timely clinical documentation and patient record maintenance are essential to ensure the continuity and quality of care. You will also attend educational meetings and coordinate department in-services to continuously improve care standards. Maintaining safety and infection control through proper application of universal precautions is critical in this patient-facing role. Strong communication with the Physical Therapist or Branch Director regarding equipment issues, supply needs, or changes in patient or staff status is expected to ensure smooth operation and optimum patient care.

The role demands a current state license to practice as a Physical Therapy Assistant, valid driver’s license with insurance, CPR certification, and reliable transportation. Experience working collaboratively in multidisciplinary healthcare teams and understanding the unique physical, emotional, social, and biological needs of the geriatric population are crucial. Fluency in English is required to effectively communicate and document patient care activities. Preferred candidates will bring at least one year of prior experience as a Physical Therapy Assistant.
