Job Description
CenterWell Home Health is a trusted leader in providing personalized, comprehensive home healthcare services that focus on patient recovery, rehabilitation, and chronic condition management. As part of Humana Inc., a prominent healthcare company, CenterWell Home Health is dedicated to delivering compassionate, high-quality care directly to patients in the comfort and convenience of their own homes. The organization brings together a multidisciplinary care team that includes nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers. This collaborative approach ensures that patients receive coordinated, functional care that improves their health outcomes and overall quality of life. \n\nCenterWell Home Health's mission is centered on helping individuals regain strength, mobility, and independence by providing targeted therapies and support designed to address their unique needs. As a home healthcare provider, the company specializes in treating senior patients and others recovering from injury, illness, surgery, or hospitalization. By focusing on whole-person wellness, CenterWell Home Health strives to enhance physical, emotional, and social well-being through its comprehensive services and community-oriented care model. The organization’s commitment to excellence is reflected in its adherence to industry-leading safety protocols, including infection control and universal precautions, and its participation in Humana's tuberculosis screening and driver safety programs to ensure patient and staff safety. \n\nThe role of a Home Health Physical Therapist Assistant at CenterWell Home Health is critical in delivering hands-on care to patients within their home environment. Reporting to the Clinical Manager, the Physical Therapist Assistant supports patient recovery by administering a variety of therapeutic treatments as prescribed by licensed Physical Therapists. These treatments may include manual therapeutic exercises, therapeutic massage, traction, and the use of heat, light, cold, water, and electrical modalities. This position requires the PTA to have a solid understanding of physical therapy techniques, excellent clinical documentation skills, effective communication with healthcare teams, and a compassionate approach to patient care. The PTA is responsible not only for treatment delivery but also for monitoring patient progress, documenting clinical records promptly, and addressing any equipment or supply needs. A foundational knowledge of the geriatric population’s physical, emotional, social, and biological changes is vital to successfully engaging patients and tailoring interventions to their individual abilities and limitations. \n\nThis is a full-time position, typically scheduled for 40 hours per week, with compensation offered on a pay-per-visit basis ranging from $41.00 to $57.00, translating to an estimated annual salary of $64,000 to $87,500. CenterWell Home Health values ongoing education and professional development, encouraging employees to attend educational meetings and department in-services to maintain high standards of care. With a workplace culture emphasizing respect, teamwork, and patient-centered care, this opportunity offers physical therapist assistants the chance to make a meaningful difference in patients’ lives while enjoying a supportive and dynamic work environment.
