The position of Home Health Licensed Practical Nurse (LPN) at CenterWell Home Health offers an opportunity to be part of this compassionate and patient-centered community. CenterWell offers competitive compensation for the Home Health LPN position, with pay ranging from $33.00 to $47.00 per visit/unit and an annual base salary between $53,000 and $73,000 for full-time employment of 40 hours per week. Additionally, CenterWell provides a $5,000 sign-on bonus to attract qualified candidates. Job Duties

  • Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN
  • Implement individualized care plans in collaboration with the patient, family, and healthcare team
  • Educate patients and their families on disease management, treatment options, and self-care techniques
  • Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes
  • Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes
  • Participate in care coordination activities and discharge planning
