The Clinical Manager role at CenterWell Home Health is a pivotal leadership position responsible for overseeing and supporting a team of clinicians delivering compassionate care in the home setting. Reporting to the Branch Director, the Clinical Manager ensures operational excellence across clinical services, regulatory compliance, and documentation quality while fostering a supportive environment that empowers clinicians and enhances patient outcomes. This role requires onsite presence at the branch office, where the Clinical Manager coordinates patient referrals, admission assessments, clinician assignments, and individualized care plans that meet rigorous agency and Medicare standards.

This position also demands active involvement in staff management including hiring, training, evaluation, coaching, and corrective actions to maintain top clinical performance and adherence to quality standards such as OASIS documentation and CMS PDGM billing policies. The Clinical Manager plays a critical role in facilitating communication among care teams, physicians, and caregivers to support care coordination, discharge planning, and optimal patient outcomes. Additional responsibilities include participating in organizational quality improvement initiatives, budget oversight, marketing efforts, and community relationship development.

Working hours are scheduled Monday through Friday, 8 a.m. to 5 p.m. Eastern Time, with on-call duties as needed. The role requires a minimum of two years as a Registered Nurse with at least one year of supervisory experience in a home health environment. A Bachelor of Science in Nursing (BSN) and current RN state licensure are mandatory, as well as valid CPR certification and reliable transportation. Salary ranges from $77,200 to $106,200 annually, with eligibility for an incentive bonus program based on company and individual performance. The Clinical Manager may also provide direct patient care in exceptional cases and assume Branch Director responsibilities in their absence.

Job Requirements

  • Graduate of an accredited school of nursing
  • BSN required (NY state requirement)
  • Current state license as a registered nurse
  • Proof of current CPR
  • Valid driver's license, auto insurance, and reliable transportation
  • Two years as a registered nurse with at least one year of management experience in a home health environment
  • Certified home health agency experience required (NY state requirement) or four years CHHA experience and six credit hours toward BSN
  • OASIS experience required

Job Duties

  • Oversee clinical operations including patient care delivery, staff management, documentation quality, and regulatory compliance
  • Review referrals and determine admission appropriateness
  • Assign clinicians and ensure plans of care meet patient needs and agency standards
  • Guide, support, and educate clinicians on goal setting, care planning, and clinical decision making
  • Ensure clinical documentation, audits, and billing comply with Medicare and payer standards
  • Participate in hiring, training, performance evaluation, coaching, and corrective action for clinical staff
  • Coordinate communication among physicians, team members, and caregivers to support care coordination and discharge planning
  • Conduct ongoing staff education based on documentation review and performance improvement data
  • Participate in quality improvement, data tracking, budgeting, marketing, and community relationship development
  • Provide direct patient care in exceptional circumstances
  • Act as Branch Director in their absence
  • Perform additional supporting tasks for clinical operations and organizational goals
