The role of Clinical Manager at CenterWell Home Health is an exciting and impactful leadership position reporting directly to the Branch Director. In this role, you will lead and support a team of skilled clinicians dedicated to delivering compassionate, high-quality care within the home setting. Your leadership responsibilities include overseeing clinical operations for the location, ensuring patient care delivery aligns with regulatory and company standards, managing staff, and maintaining the quality of clinical documentation and billing. An essential aspect of this role is guiding clinical practice and supporting clinicians in goal-setting, care planning, and clinical decision-making. You will also be responsible for coordinating communication between physicians, team members, and caregivers to facilitate effective care coordination, discharge planning, and optimal patient outcomes.

As a Registered Nurse Clinical Manager, you will ensure that referral reviews, admission appropriateness, and clinician assignments are handled efficiently. You will also play an active role in hiring, training, performance evaluation, coaching, and corrective actions for clinical staff, fostering a strong and capable team. Additionally, conducting ongoing staff education based on documentation and utilization review findings is key to maintaining excellent care standards. The position requires participation in quality improvement initiatives, data tracking, budgeting, marketing, and community relationship development. In exceptional circumstances, you may provide direct patient care and step in as acting Branch Director when necessary.

This position is full-time, typically scheduled for 40 hours per week, with an estimated salary range of $77,200 to $106,200 annually. The role offers a $10,000 sign-on bonus and eligibility for a bonus incentive plan based on company and individual performance. Job Duties

  • Oversee clinical operations for the location including patient care delivery staff management documentation quality and regulatory compliance working onsite in office
  • Review referrals determine admission appropriateness assign clinicians and ensure plans of care meet patient needs and agency standards
  • Guide support and educate clinicians help goal set care planning and clinical decision making and remain available during operating hours for clinical support
  • Ensure clinical documentation audits and billing meet Medicare payer and company standards monitor case management quality and outcomes
  • Participate in hiring training performance evaluation coaching and corrective action for clinical staff
  • Conduct ongoing staff education based on documentation review utilization review findings and performance improvement data
  • Coordinate communication among physicians team members and caregivers to support care coordination discharge planning and outcome achievement
