Job Description
Concierge Home Care is a respected home health care provider operating primarily in Jacksonville, FL, and surrounding areas. Dedicated to making a meaningful difference in the lives of patients, the agency specializes in delivering compassionate, personalized in-home care services. Their mission, "Caring for people who care for people," reflects a deep commitment to supporting both patients and the caregivers who help them. As a company, Concierge Home Care fosters a supportive and collaborative environment where caregivers can thrive professionally and personally. This organization values flexibility, autonomy, and professional growth, making it an ideal workplace for those passionate about making a real impact on the health and well-being of individuals in their communities. Concierge Home Care offers a PRN (as-needed) employment type with a pay-per-visit compensation model, providing caregivers with flexibility and control over their work schedule. Mileage reimbursement is also provided in accordance with company policy, further supporting caregivers in their essential duties.
The role of a Home Health Aide (HHA/CNA) at Concierge Home Care is critical to ensuring patients maintain dignity, independence, and comfort in their own homes. Home Health Aides provide hands-on assistance with personal care tasks such as bathing, grooming, and hygiene, as well as supporting patients with daily living activities. This role involves close collaboration with a clinical team, including experienced Registered Nurses and leadership, offering both field independence and strong clinical support. Unlike traditional nursing shifts, the HHA position involves daily route-based visits rather than extended shift work, allowing for better work-life balance. The role demands observational skills to report changes in patient condition, strict adherence to established care plans, and accurate documentation of patient visits according to regulatory and company standards. Working as a Home Health Aide at Concierge Home Care means more than just patient care; it is an opportunity to join a team-oriented, supportive culture that provides structured onboarding, ongoing clinical supervision, and opportunities for career advancement within the home health field. Caregivers benefit from access to streamlined documentation tools, supportive leadership, and employee referral bonuses. This position is ideal for individuals seeking autonomy in their work, consistent patient interaction, and meaningful contributions to patient health in a flexible and respected home care setting. If you have completed a Home Health Aide training program that meets Medicare Conditions of Participation, hold a valid driver's license with reliable transportation and auto insurance, and possess strong communication and interpersonal skills, this role invites you to join a caring and dynamic team devoted to excellent home healthcare delivery.
