As a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) at Concierge Home Care, you will play a vital role in providing personal care and hands-on support to patients in their homes, helping them maintain their independence and improve their quality of life. You will be responsible for delivering essential services such as bathing, personal hygiene assistance, and supporting patients with ambulation and exercises, ensuring their comfort and safety throughout each visit.

In addition to delivering direct patient care, you will be expected to maintain accurate and timely visit records in accordance with agency policies, ensuring all documentation reflects the patient's current condition. You will also serve as a critical link between the patient and the supervising registered nurse by promptly reporting any changes in the patient’s health status. This role extends beyond direct care, as you may also be tasked with performing essential household tasks that support the overall health care plan, contributing to a holistic approach to patient well-being.

Job Requirements

  • Home Health Aide training completion and competency evaluation

  • Reliable transportation if required by position

Job Qualifications

  • Successfully completed a Home Health Aide training program and competency evaluation as required by Medicare Conditions of Participation
  • Demonstrates a compassionate and caring attitude toward the sick and elderly
  • Ability to follow directions, read, and write effectively to carry out assigned tasks

Job Duties

  • Provide personal care, bathing, and hands-on care as outlined in the patient’s plan of care
  • Complete visit records promptly and accurately according to Agency policy
  • Report any changes in the patient’s condition to the supervising RN
  • Perform essential household tasks to support health care in the home as assigned
  • Assist patients with ambulation and exercises as directed by the care plan
