Job Requirements
- High school diploma or GED required
- Completion of federally compliant training program as required by law
- Successful completion of a competency evaluation program
- Documentation of one year employment as an aide in an institution, home care or hospice setting
- Meets agency hiring criteria including provision of professional references
- Ability to work with minimal supervision
- Responsibility for maintaining skills and adherence to policies
- Ability to demonstrate organizational core competencies and ethics
- Current driver's license and reliable transportation to and from work site

Job Duties
- Follows care plan developed by RN
- Reports client changes in condition to RN, as described on care plan
- Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities
- Assists clients with household tasks including planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding
- Measures and records client's oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered
- Performs range of motion and other simple procedures as an extension of therapy services as assigned
- Assists clients in self-administration of medication under RN supervision and documentation
