The Health & Wellness Navigator plays a vital role within the senior living community, focused on fostering trusted relationships with residents and their families while serving as a dependable resource for health and wellness guidance. This position is integral to supporting residents on their journey to achieve their highest possible level of health and well-being through a holistic approach based on the 8-Dimensions of Health & Wellness model. This model encompasses intellectual, occupational, social, environmental, emotional, physical, and health services dimensions, empowering residents to maintain an independent and fulfilling lifestyle within the community.

A critical component of this position is leading the Radar Meeting, where strategies are developed for optimal resident care, campus flow, and occupancy management. The Navigator also coordinates and participates in important community meetings related to resident health updates, transitions, and support services, fostering open communication and shared decision-making among all stakeholders. Emergency preparedness and response support are additional responsibilities, which include managing emergency medical information, coordinating room checks, and supporting residents and families through challenging health or psychosocial situations.

Educational programming and wellness initiatives form a key part of the Navigator's duties, as they collaborate with wellness and community life teams to deliver health-focused events, clinics, and support groups aimed at promoting prevention, healthy lifestyles, and successful aging. This role requires a health professional with strong organizational and interpersonal skills, capable of motivating residents toward independence while addressing complex care needs with sensitivity and ethical judgment.

Job Requirements

  • Education level of LPN or RN from an accredited college
  • Three to five years of relevant healthcare experience preferably in senior living
  • Strong organizational and care coordination skills

  • Ability to interact professionally with residents, families, and staff including challenging situations
  • Use of personal computer and general office equipment
  • Willingness to comply with personal protective equipment requirements

Job Qualifications

  • Three to five years of experience working in senior living spaces including Independent Living, Assisted Living, and Memory Care or other healthcare settings
  • Graduate of an accredited college with LPN or RN
  • Bachelor’s degree in social services, geriatrics, nursing, or related senior living services preferred
  • Strong interpersonal skills including ability to motivate and encourage residents
  • Effective problem-solving and ethical decision-making skills
  • Excellent verbal and written communication skills for diverse populations including persons with disabilities and dementia
  • Strong organizational skills including delegation, managing multiple priorities, and time management
  • Ability to collaborate with community management, internal and external resources, and communicate effectively with residents, families, physicians, and staff
  • Participation in continuing education and mandatory educational programs

Job Duties

  • Meets the standards of the Health and Wellness Navigation Program and strives to achieve bronze, silver, and gold certification
  • Oversees cross-functional departmental support of all post-acute services within the community
  • Assists residents to transition within the continuum of care services to the appropriate levels of care offered within the community
  • Leads the Radar Meeting and collaborates with the multi-disciplinary team to strategize for optimal resident outcomes, campus flow, and occupancy
  • Strives to achieve elevated levels of resident satisfaction
  • Coordinates and attends relative community meetings related to resident transitions, status updates, changes in resident condition, support services required, and appropriate level of care
  • Encourages resident participation in community programs and social events
  • Assists residents, family, and community to prepare for and provide support during emergency situations
  • Maintains emergency medical information for each resident, updating annually and assuring confidentiality
  • Coordinates new resident orientation in collaboration with Marketing and Wellness departments
  • Reviews emergency call notes, incident reports, and concern forms to implement risk reduction strategies
  • Serves as a resource for supportive strategies to improve health, function, and lifestyle
  • Coordinates assessments, offers consultations, and aids with coordination of internal and external resources
  • Supports clinical care coordination including blood pressure checks and wellness consultations
  • Initiates interactions with residents and families for issue resolution and risk reduction efforts
  • Coordinates communication with physicians, families, and staff regarding resident status
  • Oversees admission and discharge to/from other healthcare provider agencies
  • Coordinates completion and review of Independent Living Health & Well-being Questionnaires
  • Develops and maintains listings of available health-related and personal service providers
  • Partners with Wellness and Community Life Services to plan and implement health programming and education
  • Networks and contracts with qualified educators, instructors, and health professionals
  • Reviews daily check-in log for Independent Living residents and coordinates room checks
  • Collaborates with Manor Care leaders to develop objectives and policies related to resident health and wellness
  • Maintains required records, reports, studies and assures confidentiality
  • Other duties as assigned
