The 101st Place position is designed to provide direct support to individuals within a residential or periodic program setting. This non-exempt, hourly role offers flexible employment options, with full-time or part-time schedules available. The typical work schedule for this role is Monday through Friday, from 6 AM to 10 AM, with a competitive wage of $21 per hour. The role requires candidates to have prior experience working in group home settings, particularly with individuals who have high medical needs and behavioral challenges. Additionally, candidates must hold a valid driver’s license for at least one year post-permit period and maintain a clean driving record that complies with Sevita’s insurability policy.

As a Direct Support Professional, the individual will be responsible for delivering personalized care and support in alignment with individualized service or program plans. These plans encompass a wide range of support areas including socialization, health maintenance, medication administration, skill development, and behavioral improvement. The position involves assisting with daily living activities such as personal care, housekeeping, recreational participation, transportation, community integration, shopping, and citizenship activities. Accurate documentation and record-keeping are critical components of the role to ensure that all progress and changes in service plans are effectively communicated and confidentiality is maintained.

Other key functions include managing individuals’ assets responsibly within budget limits, establishing and maintaining professional relationships with clients, families, guardians, and case managers, and adhering to robust reporting standards for any incidents or concerns related to abuse, neglect, or medical issues. The role emphasizes the importance of respecting clients' rights, maintaining confidentiality, practicing universal precautions, and supporting individuals in exercising their rights.

Health care responsibilities are integral to the role and may involve accompanying individuals to medical appointments, assisting with medication administration or self-administration, monitoring health status, and ensuring proper use and reporting on medical supplies and equipment. Meal preparation support, including accommodating special diets, is also part of the job.

This position demands dependable attendance and punctuality, collaboration with team members, active participation in training and staff meetings, and a commitment to workplace safety. Maintaining a safe, clean, and hazard-free environment is essential, and the employee may assist with minor maintenance tasks and safe transportation of individuals in accordance with state and local regulations.

Candidates must meet minimum knowledge and skill requirements including a high school diploma or equivalent, preferably with six months of experience in human services, be at least 18 years old, have working knowledge of computers, and possess a valid driver’s license in good standing. Employees must undergo required state training and certification, submit to background checks and drug testing, and demonstrate proficiency in all required training areas.

  • able to exert up to 100 pounds of force occasionally and up to 50 pounds frequently
  • must submit to criminal background checks and be approved prior to working with individuals served

Job Duties

  • Provide direct support or assistance in accordance with individual service or program plans
  • assist with socialization and behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living
  • accurately document progress and activity, review records and logs to stay abreast of changes in service plans, maintain confidentiality
  • make cost-effective and appropriate purchases within the budget of the individual served and according to their choice or preference
  • maintain healthy and professional relationships with individuals, friends, families, guardians and case managers, greet family members and other visitors, implement Sevita Customer Service Standards
  • report any instance of alleged abuse or neglect according to internal and external standards, report medical, behavioral and other incidents in accordance with Sevita policy and external requirements
  • maintain confidentiality, respect the rights of persons being served, practice universal precautions, assist individuals in exercising their rights
  • accompany individuals to medical appointments as needed, relay orders and information to and from medical providers
  • accurately administer or support self-administration of medication and document delivery, promptly report administration errors, maintain appropriate security of controlled medications and supplies
  • monitor individual’s health, document health concerns, communicate with nurse or supervisor as appropriate, call 911 in emergencies, follow individual health care directives
  • correctly follow procedures in utilizing medical equipment, report malfunction or disrepair
  • prepare or support individuals to prepare food in accordance with planned menus and individual choice, know special diets and prepare individual meals accordingly, monitor mealtime and implement dining plans
  • attend orientation and ongoing training, participate in monthly staff meetings
  • work scheduled hours, arrive for work on time, stay on shift until replacement arrives, notify supervisor of any scheduling issues, fill in for other shifts as required without causing unapproved overtime or in emergencies
  • work cooperatively with others, value contributions, communicate openly, listen and show respect
  • comply with safety policies and procedures, report unsafe hazards, participate in safety training
  • safely transport individuals in community, ensure proper use of safety equipment, report accidents and safety concerns immediately
  • perform daily and seasonal housekeeping, maintain neat, clean and safe environment, report hazards
  • monitor environmental safety, make minor repairs and yard maintenance, report equipment or facilities needing repairs
  • check water temperature when assisting with bathing, participate in safety drills, protect persons in emergencies
  • perform other duties as required
