At ChenMed, the Care Facilitator role is a critical frontline position responsible for serving as an initial point of contact and patient advocate. Care Facilitators play a vital role in delivering exceptional customer service and building trusted relationships with patients. This position involves greeting patients, managing patient check-ins and check-outs, scheduling appointments, and providing essential administrative support within the medical clinic. The ideal candidate will demonstrate a strong commitment to patient care and superior service, ensuring a welcoming and organized clinic environment that reflects ChenMed's high standards.
This role demands the use of various computer programs and systems to input patient data accurately, verify insurance, coordinate appointments with both primary care physicians and specialists, and assist in managing transportation logistics such as arranging Uber or Lyft rides for patients. Care Facilitators also play a key role in patient engagement by conducting follow-up "love" calls, mailing greeting cards, and collecting customer service feedback to enhance patient satisfaction. Attending daily and weekly huddles to report on patient needs and clinic metrics like HEDIS (Healthcare Effectiveness Data and Information Set) is an important part of the job. The role requires a positive, professional attitude, strong communication skills, and excellence in multi-tasking within a fast-paced healthcare setting.
ChenMed offers competitive hourly pay ranging from $13.90 to $19.83, reflective of the candidate’s experience and credentials. Job Requirements
- High school diploma or equivalent
- Minimum of 1 year experience in medical clinic or similar environment
- Proficiency with computer systems and basic IT software
- Ability to communicate professionally with patients and colleagues
- Detail-oriented and accurate in record-keeping
- Ability to maintain patient confidentiality
- Positive attitude and strong listening skills
- Fluent in English
- Willingness to travel locally and regionally up to 10%
Job Qualifications
- High school diploma or equivalent
- Graduation from nationally accredited Medical Assistant program preferred
- Minimum of 1 year work experience in a medical clinic or similar environment
- Knowledge of medical insurance and electronic medical record (EMR) systems
- Exceptional customer service and communication skills
- Proficiency in Microsoft Office Suite including Excel, Word, and Outlook
- Ability to maintain confidentiality of patient information
- Positive and professional attitude
- Fluent in spoken and written English
- Ability and willingness to travel locally and regionally up to 10%
Job Duties
- Interact appropriately with patients and record information during check-in and check-out
- Conduct lobby checks to ensure organization and cleanliness
- Serve as gatekeeper for PCP and specialist schedules ensuring appointments for top patients
- Utilize computer systems to manage patient information, schedule appointments, and generate new patient packets
- Conduct follow-up "love" calls and send greeting cards to patients
- Answer telephones, take messages, and direct inquiries appropriately
- Verify patient insurance, collect copays, and assist with scheduling follow-up and specialist referrals
- Escort patients to exam rooms and ensure their comfort
- Act as HEDIS champion by tracking patient needs and hospitalizations
- Present customer service surveys and escalate issues for immediate recovery
- Participate in staff huddles to provide updates on patient status
- Manage transportation logistics including medication deliveries and ride scheduling
