At ChenMed, the Care Facilitator role is a critical frontline position responsible for serving as an initial point of contact and patient advocate. Care Facilitators play a vital role in delivering exceptional customer service and building trusted relationships with patients. This position involves greeting patients, managing patient check-ins and check-outs, scheduling appointments, and providing essential administrative support within the medical clinic. The ideal candidate will demonstrate a strong commitment to patient care and superior service, ensuring a welcoming and organized clinic environment that reflects ChenMed's high standards.

This role demands the use of various computer programs and systems to input patient data accurately, verify insurance, coordinate appointments with both primary care physicians and specialists, and assist in managing transportation logistics such as arranging Uber or Lyft rides for patients. Care Facilitators also play a key role in patient engagement by conducting follow-up "love" calls, mailing greeting cards, and collecting customer service feedback to enhance patient satisfaction. Attending daily and weekly huddles to report on patient needs and clinic metrics like HEDIS (Healthcare Effectiveness Data and Information Set) is an important part of the job. The role requires a positive, professional attitude, strong communication skills, and excellence in multi-tasking within a fast-paced healthcare setting.

ChenMed offers competitive hourly pay ranging from $13.90 to $19.83, reflective of the candidate’s experience and credentials. This position requires at least one year of experience in a medical clinic or similar environment, a high school diploma as a minimum educational requirement, and preferably graduation from an accredited Medical Assistant program. Basic Life Support (BLS) certification is also preferred but not mandatory.
