The Field Care Manager, Behavioral Health position is a critical role within Humana's Illinois (IL) Medicaid team. This full-time remote role requires up to 75% travel to member homes and region-based facilities across Illinois. Reporting directly to the Manager of Care Management, the Field Care Manager acts as the primary point of contact for members experiencing behavioral health challenges. The role involves delivering integrated, high-quality care coordination and support with a focus on improving behavioral health outcomes. The Field Care Manager engages members face-to-face and telephonically, taking a person-centered, holistic approach to manage and resolve complex behavioral health and social barriers.
Key responsibilities include utilizing evidence-based behavioral health interventions, crisis management, peer support, and partnering with medical and behavioral health providers to facilitate seamless, culturally competent care. The role focuses on serving diverse populations including children, adolescents, adults with serious mental illness (SMI), serious emotional disturbance (SED), substance use disorders (SUD), and justice-involved individuals. The Field Care Manager drives interdisciplinary care team efforts, coordinates care transitions, facilitates communication among providers and care management extenders, and proactively supports the member’s health literacy and social needs.
Humana offers a starting salary range of $65,000 to $88,600 annually for this position, with eligibility for bonus incentives tied to performance. The company also provides a $5,000 hiring bonus paid in two installments - $2,500 after six months and $2,500 after one year of employment. Educational benefits include 100% upfront tuition payment and reimbursement for qualifying non-tuition expenses up to $5,000 annually. Job Requirements
- Must reside in Illinois
- active Illinois license in LCSW, LMFT, or LCPC (no supervisees or provisional licenses)
- 2+ years of post-degree clinical experience in behavioral health setting
- case management experience with complex SMI, SUD, SED population
- ability to travel to region-based facilities and homes for face-to-face assessments
- ability to use various electronic information applications/software including electronic medical records
- intermediate to advanced computer skills including Microsoft Word, Outlook, and Excel
- valid driver's license, car insurance, and reliable transportation
Job Qualifications
- Active Illinois license in LCSW, LMFT, or LCPC (no supervisees or provisional licenses)

- 3+ years of in-home assessment or care coordination experience preferred

- knowledge of community health and social service agencies and resources preferred
- previous managed care experience preferred
- bilingual preferred
Job Duties
- Utilize high-quality, evidence-based behavioral health services through personalized care coordination, crisis intervention, peer support, and collaboration with medical and behavioral health providers
- provide comprehensive, integrated support to members experiencing behavioral health conditions including children, adolescents, adults with serious mental illness (SMI) and serious emotional disturbance (SED), substance use disorders (SUD), and justice-involved members
- engage members in their own communities through face-to-face meetings to build trust and facilitate care coordination
- coordinate behavioral health and medical services ensuring appropriate provider engagement and adherence to treatment plans
- improve member's health literacy while addressing health-related social needs
- serve as the driver of the member's interdisciplinary care team (ICT), overseeing care planning, transitions, and service delivery
- facilitate ICT meetings ensuring communication among providers, service coordinators, and care management extenders
- proactively support transition of care efforts
- work autonomously but seek support when needed
- collaborate with internal departments, providers, and community-based organizations to link to appropriate services and create a seamless, culturally competent care experience
- follow processes to ensure compliance with regulatory requirements by IDHS, CMS, and NCQA
