Job Description

The Idaho Department of Health and Welfare is a reputable government agency dedicated to improving the health, safety, and independence of Idaho residents. As a vital part of Idaho's public health infrastructure, the department provides a wide array of services focused on protecting vulnerable populations and supporting community well-being across the state. The Continuum Care Bureau within the department plays a crucial role in ensuring that youth in Idaho receive the correct level of care, appropriate treatment, and necessary support to foster their development and stability within their communities. By promoting healthy family environments and preventing unnecessary foster care placements, the bureau contributes significantly to the state's social service system.

The department is currently hiring a Family Support Clinician (FSC) for the Boise Westgate office. This position is essential in fulfilling the bureau’s mission, as FSCs provide clinical support and psychoeducation to birth, foster, post adoption, and other caregivers. Their goal is to stabilize youth within their home environments, reduce placement disruptions, and assist with smooth transitions during discharge from continuum care. FSCs work collaboratively with caregivers and other professionals, utilizing targeted intervention and therapeutic strategies specifically designed to address immediate behavioral concerns and equip families with effective management tools. They also play a critical role in promoting continuity of care through coordinated efforts with case managers and treatment teams.

The role demands a compassionate and skilled clinician who is able to assess early signs of distress, intervene appropriately to prevent crises, and deliver clinical services through various platforms including in-person, telecommunication, or phone support. FSCs are knowledgeable about community resources and must use this expertise to suggest relevant, accessible services that accommodate diverse family needs. The position involves some travel within the community to facilitate engagement and support. Ensuring effective communication and documentation is vital, as FSCs routinely update case managers and supervisors regarding client progress and care transitions.

Job Requirements

  • Possess a master’s degree or higher in a direct clinical practice human services field
  • Hold licensure as LMSW, LCSW, LPC, or LMFT in accordance with Idaho codes
  • Pass a pre-employment drug test
  • Successfully complete a criminal history background check
  • Be willing to travel as required
  • Ability to work flexible hours and provide services through multiple communication platforms
  • Adhere to the Idaho Department of Health and Welfare’s policies and drug-free workplace standards

Job Qualifications

  • A master’s degree or higher in a direct clinical practice human services field such as social work, psychology, marriage and family counseling, marriage and family therapy, psychosocial rehabilitation counseling, psychiatric nursing, or a closely related field
  • Licensure in a professional specialty area: Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Marriage and Family Therapist (LMFT) as per Idaho state codes
  • Demonstrated experience in providing clinical support and psychoeducation to families and youth
  • Strong knowledge of community resources and ability to coordinate care collaboratively with case managers and treatment teams
  • Excellent communication and documentation skills
  • Ability to provide services in various formats including in-person, telecommunication, and phone support
  • Commitment to maintaining confidentiality and professional ethics

Job Duties

  • Provide brief clinical support and psychoeducation to birth, foster, post adoption and other identified caregivers by offering targeted sessions that address immediate concerns and equip them with tools to manage youth behavior effectively
  • Assist in maintaining stability within their community placements by identifying early signs of distress and intervening with strategies to mitigate potential disruptions (in person, telecommunication, or phone support)
  • Knowledge of community resources to suggest services in home, office, or virtual settings to maximize accessibility and accommodate the diverse needs of families (in tandem with regional Case Manager and Intensive Care Coordinator)
  • Deliver clinical support and psychoeducation to birth, foster, post adoption and other identified caregivers during youth transitions from continuum care by ensuring they understand behavioral expectations and best practices for a smooth reintegration
  • Focus on stabilizing youth post-discharge to prevent re-entry into continuum care through structured check-ins and reinforcement of coping mechanisms
  • Coordinate with the discharging treatment team to ensure a ‘soft’ handoff and continuity of care by facilitating a collaborative transition plan that aligns with the youth’s therapeutic goals (working together with DHW Case Manager to coordinate and collaborate for youth/family needs)
  • Dependent upon age, meet with youth to assess treatment readiness and overall treatment needs through structured assessments and goal-setting discussions
