The role of the Advanced Practice Clinician (APC) within the Jump Start Assessment (JSA) program plays a crucial part in this mission. This physician-led and APC-driven initiative prioritizes clinical and home-based care delivery to patients with complex and chronic care requirements. The Advanced Practice Clinician functions as an integral member of an interdisciplinary healthcare team, engaging in in-home and virtual patient assessments to develop tailored care plans aimed at optimizing health outcomes while managing healthcare costs effectively. The position involves conducting comprehensive assessments, including detailed medical histories, physical examinations, medication reviews, and cognitive, depression, and safety screenings. These assessments typically last between 45 to 60 minutes and are conducted in the patient’s home or outpatient settings, emphasizing comfort, dignity, and personalized care.

In this role, the Advanced Practice Clinician ensures continuity of care by communicating assessment findings to primary care providers to address gaps in treatment or follow-up needs. The APC also provides education to patients, families, and caretakers on monitoring and follow-up care, including end-of-life discussions, which require empathy and expertise. Additionally, the clinician facilitates laboratory specimen collection in-home when appropriate and utilizes knowledge of community resources to refer patients for additional services, supporting holistic patient care. Compliance with all HIPAA regulations and safeguarding protected health information is a fundamental responsibility in this position.

The ideal candidate will possess an active California state Nurse Practitioner or Physician Assistant license, with board certification and necessary credentials such as an NPI number, DEA registration, and valid BLS certification. Job Requirements

  • Master’s degree from an accredited NP program or PA program
  • Active California state Nurse Practitioner or Physician Assistant license
  • Active RN license and furnishing number (NP only)
  • Nurse Practitioner Board Certification or Physician Assistant Certification
  • NPI number, DEA registration, valid BLS certification
  • Valid California driver license and current automobile insurance
  • Ability to communicate positively, professionally, and effectively with others
  • Effective problem-solving, organizational, and time management skills
  • Experience with older adult (geriatric) patients preferred
  • Previous clinical or home care experience preferred
  • Compliance with HIPAA regulations
  • Comfort with end-of-life care discussions
  • Ability to perform physical tasks including standing, walking, and lifting up to 20 pounds

Job Duties

  • Conduct in-home assessments with some outpatient clinical work on health plan members
  • In-home assessments including comprehensive history, physical exam, medication review, and cognitive/depression/safety screenings
  • Comfort with end-of-life care discussions
  • Laboratory specimen collection in the home setting when appropriate
  • Identify diagnoses for active medical and care management focusing on chronic disease management
  • Communicate assessment findings to inform the PCP of potential gaps in care
  • Provide education to patients, families, and caretakers with emphasis on monitoring and follow-up needs
  • Knowledge of appropriate community resources for referral
  • Comply with all HIPAA regulations and maintain security of protected health information
