Job Description

This position is a non-exempt, float role within REM Community Services requiring open availability to work days, evenings, overnights, and weekends. It notably includes travel between various metro area programs with a wage of $23.25 per hour. The job entails delivering direct support services to individuals in either residential or periodic program settings, typically supporting 4 to 6 individuals in residential environments or up to 10 in periodic programs. Responsibilities include assisting individuals with daily living tasks such as personal care, housekeeping, transportation, and recreational activities. The role is essential for implementing service plans that cover socialization, health maintenance, medication administration, and skill development. Caregivers play an integral role in ensuring the safety, dignity, and well-being of individuals while fostering community integration and citizenship.
Candidates must have a minimum of three years’ experience as a caregiver and a valid driver's license maintained for at least one year post permit with a clean motor vehicle record that complies with Sevita's insurability policy. The position requires a high school diploma or equivalent and some experience in human services. Employees must partake in ongoing training and contribute positively to a team environment. They will also be responsible for documenting progress accurately, reporting any incidents or concerns, and adhering to professional standards, including confidentiality and respect for individuals' rights. This role demands compliance with state-required training and certifications, adherence to safety and regulatory standards, and a commitment to workplace safety and teamwork. With no supervisory responsibilities, this role is essential in frontline care and support.

Job Requirements
- must be 18 years of age
- valid driver’s license for at least one year post permit
- clean motor vehicle record that meets Sevita insurability policy
- valid driver’s license in good standing
- car registration and vehicle insurance if providing transportation
- ability to complete state-required training and certifications
- pass criminal background checks including checks against Central Registry of Offenders Against Individuals with Developmental Disabilities and Child Abuse Registry Information
- submit to drug testing prior to and during employment
- ability to exert up to 100 pounds of force occasionally and 50 pounds frequently
- ability to work days, evenings, overnights and weekends with flexible schedule
- willingness to travel to different program locations throughout metro area
Job Qualifications
- high school diploma or equivalent preferred
- six months of experience in human services preferred
- three years of caregiver experience
- working knowledge of computers
- valid driver’s license in good standing
- completion of all state-required training and certifications in mandated timeframes
- ability to maintain confidentiality
- excellent communication and interpersonal skills
Job Duties
- Provide direct support or assistance per individual service or program plans
- assist with socialization and behavioral development
- assist with personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other daily living activities
- accurately document progress and maintain confidentiality
- report any instances of abuse, neglect or incidents according to policy
- maintain professional relationships with individuals, families and case managers
- prepare or support meal preparation and follow dietary plans
- attend training and participate in staff meetings
- follow all workplace safety policies and participate in safety drills
- transport individuals safely and ensure use of safety equipment
- perform daily housekeeping and minor maintenance tasks
- assist individuals with exercising their rights and maintain confidentiality
