The Coordinator of IDD Provider Services role plays a pivotal part in overseeing the daily operations of Individualized Skills and Socialization sites (ISS), including contracted ISS locations alongside services in community settings, residential homes, nursing facilities, and host home services. This position ensures that services offered comply with established regulations and quality standards, maintaining a seamless and effective continuum of care for individuals served. Acting as a liaison with treatment teams and Waiver Provider Coordinators, the Coordinator facilitates communication and coordination to support client-centered care plans, monitoring service implementation and compliance rigorously.

Beyond program oversight, the Coordinator handles financial account management for consumers represented as payees, ensuring timely paperwork submission for maintaining eligibility. The role is integral to staffing operations, involving the recruitment, selection, and training of employees and contract staff across various service areas. This position also involves conducting residential monitoring assessments and inspections, adhering to safety requirements and regulatory standards. Due to 24-hour care responsibilities, the Coordinator must participate in on-call rotations and remain flexible with work hours, adapting to emergent needs beyond the typical 8-5 schedule. Regular travel within the region is required to meet operational demands. The Coordinator reports to ensure high-quality services are maintained, supporting Tri-County’s mission of delivering meaningful and compassionate care in the mental health and intellectual disabilities community.

The position is based in Conroe, TX, and offers an annual salary of $52,380. Job Requirements

  • High school diploma or GED required
  • Bachelor's Degree with relevant course work preferred
  • Two years experience working with individuals with disabilities
  • Supervisory experience preferred
  • Ability to pass physical agility test
  • Valid driver's license
  • Must be insurable through Tri-County
  • Willingness to participate in on-call rotation
  • Flexible availability beyond normal business hours
  • Ability to travel within the region

Job Qualifications

  • High school diploma or GED
  • Bachelor’s Degree preferred with coursework in social, behavioral, human services, social science, business or related field
  • Two years of experience working with individuals with disabilities
  • Supervisory experience preferred
  • Ability to pass physical agility test
  • Valid driver's license
  • Insurable through Tri-County
  • Experience and education may be substituted totaling at least six years

Job Duties

  • Serve as a liaison with members of treatment team to provide for continuity and quality of services to individuals served
  • Coordinate with Waiver Provider Coordinators to ensure assessments, implementation and monitoring of all services while ensuring compliance
  • Coordinate provider services for individuals residing in nursing facilities
  • Monitor and process financial accounts for each consumer represented as payee and ensure documents are submitted timely to maintain eligibility
  • Manage daily operations of 24-hour residential services ensuring staff availability
  • Recruit, select and train employees and contract staff for various service areas
  • Conduct Residential Monitoring Assessments and Inspections as assigned
  • Participate in on-call rotation due to 24-hour care responsibilities
  • Maintain flexible hours to assist outside normal 8-5 shift
  • Travel within the region as required
