Job Requirements
- Bachelor’s degree in a human service focus or equivalent experience
- Experience coordinating services for individuals with intellectual/developmental disabilities
- Access to reliable automobile and valid driver’s license
- Excellent communication skills
- Proficiency in Microsoft Office applications
- Ability to multitask and manage complex work processes
- Strong organizational skills
- Commitment to person-centered care philosophy
- Ability to visit clients in Baltimore region
- Willingness to work full-time telecommuting with local visits
Job Qualifications
- Bachelor’s degree in a human service focus or an Associate's degree in a human service focus with two years of human service experience or seven years of specific human service work experience
- Experience in coordinating services for individuals in Medicaid or waiver programs or coordination services for individuals with intellectual or developmental disabilities is strongly preferred
- Competency-based skills and working knowledge in negotiation and conflict management, crisis management, advocacy, problem solving, relationship building, person- and family-centered care, community resources including Medicaid, mental health, social services, juvenile justice, vocational rehabilitation, corrections
- Knowledge of COMAR regulations governing services for individuals with intellectual/developmental disabilities
- Strong belief in opportunities for people with disabilities to fully participate in the community
- Excellent communication skills including writing and speaking
- Computer skills including Word, Excel, and PowerPoint
- Ability to manage complicated work processes and multi-task
Job Duties
- Maintain a gradually developed caseload between 30-40 clients with intellectual/developmental disabilities
- Advocate on behalf of clients while supporting self-advocacy and participation in service planning
- Assist clients with the development of an appropriate and relevant Person-Centered Plan in accordance with COMAR regulations and agency policies
- Research, access and help implement services and supports in a timely manner, ensuring the most integrated settings to meet client needs
- Monitor the provision of services to clients by performing regular information gathering and research relevant to the client’s supports and services
- Visit clients in settings where supports are provided or other locations as desired by clients, at minimum four times during the client’s plan year depending on coordination intensity
- Assist with all client enrollment-related activities to ensure continuity of supports and services
- Facilitate frequent person-centered planning meetings including coordinating quarterly visits, annual PCP meetings, and interdisciplinary meetings as needed
- Conduct required assessments with clients per DDA regulations and program requirements
- Maximize clients’ participation in team process and promote opportunities for self-leadership
- Negotiate and resolve disagreements within the interdisciplinary team
- Annually advise participants of their right to choose from qualified providers
- Respond to provider-generated incident reports timely to ensure individual health and safety
- Document client services accurately to meet billing standards within Maryland’s LTSS system and The Coordinating Center’s systems
- Attend all external and internal required meetings and trainings
- Maintain professional relationships with external service providers to ensure quality coordination of care
