The Concierge Nurse Navigator role at TCP Medical Group is a vital position designed for a skilled nursing professional who thrives in a fast-paced clinical environment. This role brings together clinical expertise, operational acumen, and administrative proficiency to support both patients and healthcare providers effectively. As a Concierge Nurse Navigator, you will be responsible for managing daily clinic operations, ensuring compliance with regulatory standards, maintaining accurate financial and membership information, and providing hands-on clinical care. You will play a crucial role in facilitating workflow, managing medical supplies, and supporting providers during medical examinations and procedures.

Your patient care duties will be comprehensive, involving preparing patients for examinations, assisting during procedures, administering medications, collecting and processing laboratory specimens, and coordinating patient referrals. Additionally, this role demands excellent communication skills for triaging phone calls, documenting patient interactions, participating in interdisciplinary meetings, and contributing to quality improvement initiatives aimed at enhancing patient care outcomes.

This full-time position requires a dedicated healthcare professional with at least one year of experience as a Licensed Vocational Nurse (LVN) and a strong preference for candidates with pediatric experience and previous nurse navigation exposure in specialty areas. The role demands a high level of clinical knowledge and the ability to manage complex tasks efficiently, ensuring not only the smooth operation of the clinic but also optimal patient satisfaction and safety. Job Requirements

  • Diploma or GED and other diploma or certificate from a school of vocational nursing required or documentation from the Texas Board of Nursing verifying an exception to the requirement for a diploma from a school of vocational nursing
  • LVN - Licensed Vocational Nurses State of Texas
  • BLS - Cert-Basic Life Support American Heart Association
  • 1 year LVN experience required
  • 1 year pediatrics exp preferred
  • 1 year nurse navigation expeience in a specialty perferred

Job Qualifications

  • Diploma or GED and other diploma or certificate from a school of vocational nursing required or documentation from the Texas Board of Nursing verifying an exception to the requirement for a diploma from a school of vocational nursing
  • Licensed Vocational Nurse (LVN) certification in the State of Texas
  • Basic Life Support (BLS) certification from the American Heart Association
  • Minimum of 1 year of LVN experience
  • Preferred 1 year experience in pediatrics
  • Preferred 1 year experience in nurse navigation in a specialty

Job Duties

  • Provides daily operations
  • Monitors and ensures patient flow
  • Prepares deposit weekly and assures daily work is balanced and ready for pickup by bank courier
  • Collects and disseminates mail and deliveries per practice protocol
  • Participates in interdisciplinary meeting to promote and ensure adjustment and continuity of patient care
  • Prepares patient for examination per practice protocol
  • Completes treatments/lab as directed by provider and within scope of position
  • Triages phone calls per practice protocol and within scope of practice
  • Documents all clinical patient care calls and interventions per organization recommendations
  • Participates in quality improvement initiatives that support patient care
  • Verifies provider order and patient per practice protocol
  • Collects and processes specimen according to manufacturer’s instructions and using proper technique
  • Coordinates patient care by following up on lab results, ER visits, or any part of care that needs coordination
  • Coordinates referrals
  • Reviews billing information
  • Monitors TCP Concierge Self-Pay Workqueue on a weekly basis and corrects within seven (7) business days of delinquency
