The Community Outreach Specialist position at ClinNEXUS is a pivotal role designed to establish and grow the company’s footprint within the Monterey, Santa Cruz, and Salinas areas. This position is essential in building strong referral pathways and relationships with Medi-Cal members qualified for Enhanced Care Management (ECM) services. The specialist will blend community outreach, partner development, member engagement, and enrollment assistance to develop a sustainable and effective program presence from the ground up. The role requires significant time spent in the field at various locations such as health fairs, federally qualified health centers (FQHCs), shelters, clinics, behavioral health settings, and community events.
Success in this hands-on position means converting local relationships and community awareness into a thriving referral pipeline that supports improved healthcare coordination for vulnerable populations. Day-to-day responsibilities will include direct member engagement, eligibility screenings, referral tracking, consent acquisition, and collaboration with intake and care management teams to facilitate enrollments. The Community Outreach Specialist will play a critical role as part of the outreach and enrollment team, serving at times as an onsite ECM representative to ensure smooth workflows and effective communication between ClinNEXUS and partner entities.

Job Requirements
- Education equivalent to high school diploma or higher
- minimum two years’ experience in healthcare outreach or related fields
- valid California driver’s license
- reliable personal transportation
- proof of insurance for vehicle
- clean driving record
- ability to work independently in a field-based role
- strong verbal and written communication skills
- proficiency with computer platforms including Google Workspace
- ability to pass background check and drug test
- ability to undergo credentialing processes including FACIS and licensure verification
- ability to stand and walk for extended periods
- willingness to frequently travel locally within Monterey, Santa Cruz, and Salinas
- ability to maintain confidentiality and comply with HIPAA and other regulatory requirements
Job Qualifications
- Two or more years of experience in healthcare outreach, community health work, care coordination, intake, enrollment, or social services
- strong understanding of Medi-Cal/Medicaid programs including CalAIM and underserved populations
- experience managing referral pipelines with accurate and timely documentation
- ability to engage members across phone, in-person, and field settings
- ability to work independently while meeting team goals and compliance requirements
- strong relationship-building skills with community partners and diverse populations
- demonstrated problem-solving skills and comfort in evolving environments
- ability to translate community insights into growth strategies
- highly organized with excellent communication and documentation habits
- proficiency with Google Workspace, HealthCloud for Salesforce, or similar platforms
- valid California driver’s license and reliable transportation
- able to meet credentialing requirements such as background checks and drug testing
- preferred qualifications include Associate’s or Bachelor’s degree, direct ECM, CalAIM, Medi-Cal managed care experience in the region, background as a Community Health Worker, Promoter, Medical Assistant, or Case Manager, existing relationships in the Monterey/Santa Cruz/Salinas area, and Spanish fluency
Job Duties
- Establish ClinNEXUS's presence through health fairs, community events, churches, shelters, food distribution sites, and behavioral health settings
- build referral partnerships with community-based organizations, FQHCs, hospitals, clinics, behavioral health providers, social service agencies, housing organizations, and county programs
- educate community partners on Enhanced Care Management eligibility, member benefits, referral pathways, and the ClinNEXUS care model
- identify and engage potential ECM members in person, by phone, or in the field
- conduct preliminary eligibility screenings, obtain member consent, and convert qualified referrals into active enrollments in coordination with intake and care management teams
- serve as an onsite ECM representative when assigned, collaborating with discharge planners and care transition teams
- document outreach, referral, screening, and enrollment activities, maintaining organized pipeline tracking
- track and report key metrics including referral volume, conversion, partner engagement, and event activity
- conduct or support social determinants of health screenings, escalate urgent member needs, and ensure warm handoffs to care teams
- maintain confidentiality and ensure compliance with all relevant regulations and documentation requirements
