The position requires working 12-hour shifts with options for day or overnight schedules, and it provides competitive pay based on experience. Job Duties
- Review and implement activities related to Individual Support Plans and work towards plan outcomes and objectives
- perform all required documentation and ensure organizational, funder and accreditation requirements are met
- review all organizational communications each shift
- provide education, assistance, mentoring and support to individuals including bathing, personal hygiene activities, dressing, cooking according to dietary guidelines, maintaining a safe home environment, cleaning, laundry, transportation
- provide assistance, education and modeling behaviors during community outings and social opportunities
- administer medications and provide medical care according to physician orders and RN delegations
- communicate changes in functioning, health status or behaviors to support team
- accompany individuals to medical appointments and complete necessary documentation
- provide positive behavioral interventions as outlined in ISP or behavior development plan
- attend all required staff meetings and participate in ongoing education and training
